GENERAL PURPOSE: Under the direction of the Transportation Manager and a Licensed Nurse, assist with the daily transportation care of residents. Assist the Resident needing transport with daily living, supervised activities at non-residential care facilities (medical appts. for example) or special events through Activities.
ESSENTIAL FUNCTIONS: : In addition to performing Essential Functions, this position may be required to perform a combination of supportive functions based upon the requirements of the organization.
- Assist the residents with activities of daily living as necessary, including: personal grooming, oral care, back care, foot care and peri-care, bathing and toileting.
- Must have excellent customer service skills.
- Encourage self-esteem and self-care as appropriate.
- Establish caring relationships with the residents and their families by recognizing the spiritual and emotional needs of the residents and providing appropriate support.
- Observe record and report using policies, general and specific physical and mental conditions of residents.
- Take and record temperature, pulse and blood pressure, height and weight per established guidelines (as needed).
- While on transport, assist residents with meals and refreshments as necessary. Record nutritional intake of residents (if needed).
- Assist with rehabilitation of residents according to their care plans.
- Provide Residents with communication assistance, if needed.
- Consulting with the team caring for the Residents, to evaluate the Residents needs and plan for continuing services.
- Escort individual or groups and assist in transporting Residents to locations outside the home, such as to physicians offices or on outings, using facility vehicles.
- Provide residents with help walking, exercising and moving in and out of vehicles etc.
- Assist in transporting Residents to treatment units, using a wheelchair or stretcher.
- Monitor visitors activities in order to ensure compliance with establishment and safety practices.
- CPR and First Adie Certificate or in the process of obtaining within three months of hire.
- Hours varied including days, evenings, weekends, and holidays.
- All staff is required to follow all safety regulations and precautions at all times.
- Staff must ensure that their Department is maintained in a clean and safe manner by assuring that all necessary equipment and supplies are maintained.
- Report all hazardous conditions/equipment to immediate supervisor/manager
- Report all accidents/incidents to immediate supervisor/manager within 24hrs.
- All staff must wear protective clothing and equipment when handling infectious waste and or blood/body fluids or when doing Hazardous work i.e. working with chemicals, electrical or day to day maintenance repairs.
- All Staff are required to participate in Person-Centered Care - Managers/Supervisors will help staff in assisting in participating in Person-Centered Care.
