Description

GENERAL PURPOSE: Under the direction of the Transportation Manager and a Licensed Nurse, assist with the daily transportation care of residents. Assist the Resident needing transport with daily living, supervised activities at non-residential care facilities (medical appts. for example) or special events through Activities.

ESSENTIAL FUNCTIONS: : In addition to performing Essential Functions, this position may be required to perform a combination of supportive functions based upon the requirements of the organization.

  1. Assist the residents with activities of daily living as necessary, including: personal grooming, oral care, back care, foot care and peri-care, bathing and toileting.
  2. Must have excellent customer service skills.
  3. Encourage self-esteem and self-care as appropriate.
  4. Establish caring relationships with the residents and their families by recognizing the spiritual and emotional needs of the residents and providing appropriate support.
  5. Observe record and report using policies, general and specific physical and mental conditions of residents.
  6. Take and record temperature, pulse and blood pressure, height and weight per established guidelines (as needed).
  7. While on transport, assist residents with meals and refreshments as necessary. Record nutritional intake of residents (if needed).
  8. Assist with rehabilitation of residents according to their care plans.
  9. Provide Residents with communication assistance, if needed.
  10. Consulting with the team caring for the Residents, to evaluate the Residents needs and plan for continuing services.
  11. Escort individual or groups and assist in transporting Residents to locations outside the home, such as to physicians offices or on outings, using facility vehicles.
  12. Provide residents with help walking, exercising and moving in and out of vehicles etc.
  13. Assist in transporting Residents to treatment units, using a wheelchair or stretcher.
  14. Monitor visitors activities in order to ensure compliance with establishment and safety practices.
  15. CPR and First Adie Certificate or in the process of obtaining within three months of hire.
  16. Hours varied including days, evenings, weekends, and holidays.
  17. All staff is required to follow all safety regulations and precautions at all times.
  18. Staff must ensure that their Department is maintained in a clean and safe manner by assuring that all necessary equipment and supplies are maintained.
  19. Report all hazardous conditions/equipment to immediate supervisor/manager
  20. Report all accidents/incidents to immediate supervisor/manager within 24hrs.
  21. All staff must wear protective clothing and equipment when handling infectious waste and or blood/body fluids or when doing Hazardous work i.e. working with chemicals, electrical or day to day maintenance repairs.
  22. All Staff are required to participate in Person-Centered Care - Managers/Supervisors will help staff in assisting in participating in Person-Centered Care.

Qualifications

QUALIFICATIONS/BASIC JOB REQUIREMENTS: 

  • High school diploma or GED 

  • Certification as a Nursing Assistant in the State of Arizona is required. Archie Hendricks Sr. Skilled Nursing Facility is willing to assist in obtaining state certification for those who genuinely display compassion, empathy, and a willingness to care for vulnerable adults.  

  • Ability to understand and speak fluent Tohono O’odham is preferred.
