Since our founding in 1975, we’ve been deeply rooted in providing housing services to the most vulnerable and have expanded into providing permanent supportive housing, and person-centered home care in communities across Massachusetts.Â

We’re looking for a strategic Sr. Program Manager to help us lead the launch of a bold initiative to bring in-home support to hundreds recently unhoused residents living in housing first supportive housing across the Boston and South Shore area. As the driving force behind this initiative, you’ll build the program infrastructure, shape the strategy, and implement it. You'll have the opportunity to work alongside state agencies, housing providers, and community stakeholders—all aligned around a shared vision: helping people thrive in their homes, with the daily support they need.

Key Responsibilities
- Design a Scalable Model
Build an in-home services delivery program tailored to supportive housing settings—from staffing to enrollment pathways, documentation, scheduling, and compliance systems.
- Lead Program Launch
Set timelines, milestones, and metrics. Guide the program through initial implementation and early growth, ensuring we can scale with quality and efficiency.
- Collaborate with Housing & Care Partners
Cultivate strong working relationships with external care managers and support staff embedded in housing first communities. Equip them with the tools and knowledge to help residents access in-home support services.
- Build & Support the Team
Oversee key staff as the program grows—such as care managers, home health aides, and nurses. Create a positive team culture rooted in mission and accountability.
- Solve Real Challenges
Design smart, sustainable solutions to long-standing hurdles in the field—such as client engagement, physician paperwork, and caregiver retention.
- Track Progress & Adapt
Use data and feedback to improve systems, streamline operations, and monitor outcomes.
