What you will do?

As the Supportive Housing Coordinator, you will provide a holistic approach to help vulnerable individuals and families stay housed and live a more productive life in the community. The Coordinator is responsible for working with the client and the community linking them to an array of community-based services based on their specific needs. Using a model of service rooted in Trauma-Informed Care, this individual proactively engages members of the household to create and implement a plan that supports stability in housing as part of a productive life in the community.

Specific responsibilities include:

  • Effectively utilizes Trauma-Informed Care principles to thoroughly understand the neurological, biological, psychological and social effect of trauma and its impact on the clients served. 
  • Emphasizes physical, psychological, and emotional safety of the individual/family being served.
  • Empowers survivors to rebuild a sense of control and empowerment, utilizing skills to achieve health, recovery, and personal growth.
  • Works with the client to identify root causes of instability in all of life’s primary domains: education, community, family and children.
  • Develops relationships with community providers that will be resources for the clients
  • Compiles community resource packet in each community served to use in conjunction with Client Service Plan development. 
  • Monitors Client’s progress on the Service Plan, and communicates with community service providers as needed.
  • Works with the Client to adjust/increase the Service Plan to address ongoing concerns.
  • Identifies meeting frequency and uses meetings to effectively monitor Tenant progress.
  • Tracks and effectively documents meeting frequency and Tenant success/barriers/plan changes as needed.

Qualifications

What you need:

A Bachelor’s Degree plus three (3) years of relevant experience or High School Diploma and eight (8) years of relevant experience is required. Must have strong skills in working with the public and understand the family and children’s service environment both locally and on the State level.
