Identify, assess and admit appropriate patients for hospice services.

INTERPERSONAL RELATIONSHIPS:
All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.

ESSENTIAL FUNCTIONS:
- Performs admissions process. (80%)
- Accepts patient referral, visits patient in residential, nursing home or hospital setting.
- Performs assessments, i.e. vital signs, pain assessment, breath sounds, etc.
- Determines appropriateness for admissions.
- Completes documentation for admission to services if appropriate.
- Consults with supervisors, MDs, etc. regarding appropriateness for hospice services.
- Refers patient back to referral source for additional follow-up as needed.
- Teaches patient and caregivers (including nursing home staff) on end-of-life care – e.g. disease process, death and dying, comfort measures, medication use, side effects, etc.
- Initiates Plan of Care.
- Provides psychosocial support to patients and families.

- Coordinates patient care. (20%)
- Initiates coordination of personal care, DME, and obtaining medications and medical supplies.
- Documents in a timely manner patient assessments, patient contacts, physician orders and progress notes.
- Acts as a liaison between patient and physicians, other care team members and referral sources.
- Other duties as assigned.
