POSITION SUMMARY:
Under the direction of an RN Case Manager or Patient Care Manager, provides quality personal care following an established Plan of Care and performs other supportive tasks as assigned.

INTERPERSONAL RELATIONSHIPS:
All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.

DUTIES AND RESPONSIBILITIES:
- Assists with personal hygiene--bathes, grooms (hair and nails), provides oral hygiene, checks skin integrity.         Â
- Provides peri-care and performs Foley care and maintenance.           Â
- Dresses patients. Â Â Â Â
- Performs patient transfers, turns, etc. while ensuring safety of patient and self.
- Takes vital signs and weights as directed. Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â
- Ambulates patients as appropriate and does range of motion exercise as directed. Â Â Â
- May perform simple dressing changes as directed by the RN (i.e. non-sterile, non-surgical, and not greater than Stage 1 pressure ulcers).                                                                                                     Â
- Assists with medications that are ordinarily self-administered.          Â
- Provides care consistent with written instructions as outlined in patient’s care plan.                Â
- Demonstrates understanding and support in interactions with team members                                          Â
- Communicates with RN Case Manager and other team members as appropriate; reports changes in patient’s condition and needs.                                                            Â
- Documents care in a timely manner, and in compliance with organization policies and procedures.
- Obtains supplies and teaches use of medical supplies. Â Â Â Â Â Â Â Â Â Â Â
- Uses and reinforces the RN’s teaching on use of equipment.
- Checks equipment for safety and cleans equipment.              Â
- Changes linens.                                                                                       Â
- Provides nutritional support, assists with feeding, light meal preparations and light housekeeping as indicated.
