Your Role

Provides care coordination for adults with chronic complex illnesses, in the primary care setting, under minimal supervision.  This individual serves in an expanded health care role to collaborate with members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services.....

Qualifications

Education Requirements:       Associate or Bachelor’s Degree in a healthcare or social service field.
    
Minimum Experience: Previous positions in a Care Coordination capacity; 2-5 years work experience in a healthcare setting; previous experience managing patient registries preferred; previous experience working as part of a healthcare team; experience documenting in an electronic health record; experience as a participant in continuous quality improvement.

Compensation 

Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
