What You’ll Do
Lead & Empower the Care Management Team
- Supervise, mentor, a team of care managers; nurses, and social workers.
- Provide clinical oversight, case review, and decision-making support for clients aging needs.
- Establish clear workflows, best practices, and quality standards to ensure premium care.
Delegate & Optimize Operations
- Streamline care management processes to improve efficiency and team productivity.
- Oversee case assignments, documentation, and compliance to maintain top-tier service.
- Ensure care managers have the training, tools, and support they need to excel.
Client Onboarding & Care Planning
- Conduct comprehensive assessments and care planning for new clients.
- Collaborate with families, caregivers, and medical providers for seamless coordination.
Develop Policies & Drive Growth
- Refine and enhance policies, procedures, and care management strategies.
- Work alongside leadership to improve services and expand impact.
