A day in the life of this role:

Reporting to the nurse manager, the Registered Nurse at Outer Cape Health Service (“OCHS”) is responsible for providing patient care based on the nursing process within assigned work periods. The incumbent functions as a member of a multi-disciplinary team to achieve patient outcomes consistent with a comprehensive plan of care.

 

Essential Functions:

  • Manages all assigned in baskets to include but not limited to telephone encounters, outside events, overdue results, my chart communications, etc.
  • Assist with prompt triaging of patients based on organizational triage workflow
  • Based on communication with patient or clinician, develop plan and communicate to patient and clinician
  • Performs medication reconciliation in a pre-visit prior to patient arrival where indicated
  • Make appropriate referrals required for the management of patient care
  • Prioritize communication of patient’s abnormal and normal test results to clinician, and to patient (if requested)
  • Seek appropriate and timely guidance from the nurse manager depending on the level of concern
  • Provide the best patient experience possible; address patient’s concern and questions, explain procedures, testing, and diagnosis thoroughly. 
  • Always maintain patient privacy and confidentiality of information in accordance with OCHS policies and HIPAA
  • Maintain patient records with accurate and timely documentation in the Electronic Medical Record (EMR) in accordance with OCHS policies.
  • Educate patients on the proper use of medical equipment, prescribed regimens, and other nursing care regimens.
  • Refer patients to outside sources, programs, or support groups, for additional education or referrals as appropriate.
  • Promote and support teamwork by collaborating with team members including providers, Medical Assistants, and Patient Service Representatives
  • Adheres to the National Patient Safety Goals that are applicable to ambulatory care centers (including but not limited to improving the accuracy of patient identification, improving the safety of using medications, reducing the risk of care-associated infections, and following the universal protocol of preventing wrong site/wrong procedure/wrong person)
  • Experience or interest in team-based care, aligning with OCHS goal of transitioning to a team-based care model practicing multi-specialty collaboration between clinical team members to provide prompt and high-quality comprehensive care to all patients
  • Other related duties and projects, as required or assigned to ensure the needs of the health Center and its patients are met

 

Qualifications

Qualifications:

  • Bachelor of Science in Nursing from an accredited school of nursing by AACN
  • Current Registered Nurse (RN) license from the Massachusetts State Board of Registration
  • 1-year professional nursing experience (ED, urgent care, or ambulatory/primary care setting) or comparable experience
  • BLS certification required
  • Maintain and enhance professional nursing skill and knowledge by attending continuing education programs. 
  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
  • Excellent written and verbal communication skills – bi-lingual helpful
  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
  • Excellent judgment and ability to solve problems in a timely manner.
  • Ability to adhere to strict confidentiality standards.
  • Demonstrated ability to work effectively both as a team member and independently.
  • Demonstrated ability to work in a culturally diverse and inclusive environment
