Description

Reporting to the Director of Behavioral Health, the Behavioral Health School Based Clinician works in collaboration with the school-based navigator and other OCHS staff to meet the needs of patients, under the parameters of OCHS’ HRSA-sponsored program for School-Based services.  This is a hybrid position, with expectation that the clinician will be required to be present for in-person meetings and clinic-based appointments. 

Essential Functions: 

  • Provides direct services to youth and families, including information and referral, assessments, coordination of treatment, and ongoing therapeutic interventions through individual, family and/or group modalities. 

  • Works in collaboration with the school-based navigator to offer priority access to patients appropriate for tele-behavioral health services 

  • Works and collaborates with assigned team to implement joint care plans, as applicable, and balance a mix of short-term, maintenance, group, individual and extended therapeutic modalities to ensure ongoing access and best possible outcomes 

  • Ensures that patients are screened using the appropriate screening tool (CAN, PHQ-2,9) and is properly scored and that patient cared is directed accordingly 

  • Identifies and prioritizes “at-risk” patients and provides immediate intervention and follow-up referrals 

  • Practices evidence-based behavioral assessments and interventions relevant to child/adolescent behavioral health concerns 

  • Provides on-demand support for urgent situations and requiring immediate or very short-term responses 

  • Collaborates daily with healthcare team and other OCHS staff to assure that OCHS clients with behavioral health issues are treated in a coordinated and integrated fashion, moving towards the overall goal of full integration between primary care and behavioral health at OCHS 

  • Maintains case records in accordance with OCHS policies and procedures 

  • Interacts with referral sources and other internal and external sources as necessary to ensure the coordination and access to services for patients with behavioral health needs 

  • Participates in staff supervision, in-service trainings and other OCHS meetings as directed by the Director of Behavioral Health  

  • Other related duties and projects as assigned 

 

Qualifications

Qualifications: 

  • Master’s Degree in Clinical Social Work or related fields.  Licensure as an LICSW or LMHC preferred 

  • Strong clinical, organizational and case management skills; computer literate and Internet proficient 

  • Three to five years’ experience in behavioral health services 

  • Two years’ experience providing behavioral health services in a Community Health Center 

  • Bi-lingual and bi-cultural capabilities preferred 

  • Experience in Substance Use Disorder Treatment preferred
