An Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness.
This service consists of substance use services and structured rehabilitative interventions intended to increase and restore an individual’s ability to live successfully in the community.
The role of the Substance Abuse Professional is to assess and track stages of change readiness and treatment and ensure that team treatment approaches are consistent with an individual’s readiness plan.Â

Job Duties Include:
- Use cognitive behavioral approaches and relapse prevention to work with individuals in later stages of change readiness
- Facilitate the Person Centered Planning process for individuals assigned to him or her
- Show knowledge and use of clinical process to achieve positive outcomes to include: evaluation, assessment, service, treatment planning, intervention techniques, aftercare, and transition planning
- Remain available face to face or by telephone, 24 hours a day 7 days a week as staff serves as the first responder in the event of crisis situations. The service site will be at any location the consumer or family needs

Basic Requirements:
- Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
- Valid driver’s license, auto insurance coverage and reliable transportation
- Pass criminal background check and other regulatory checks
- Provide proof of your education

Education and Experience Required:
- For LCAS, LCASA - A bachelor’s degree in Human Services, Social Work, Special Education, or other closely related Human Services area
- For CADC - High School education
- One (1) year of professional experience with the adult mental health population
