Description

Healix Infusion Therapy is seeking a PRN Clinical Pharmacist for our pharmacy located in Sugar Land, TX!

 

SCHEDULE: Monday - Friday; 8 hour shifts.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Include the following. Other duties may be assigned.

  • Ensures that the pharmacy meets all state and federal laws and regulations, while working in the pharmacy
    • This is evidenced by successfully passing State Board of Pharmacy Inspections (i.e., no warning or deficiencies), Accreditation inspections, other regulatory inspections and by scores on Quarterly Audits
  • Promptly and accurately receives, interprets, evaluates, prepares, and dispenses prescription drug orders. o
    • This is evidenced by, QCR’s, Billing office discrepancies, and Quarterly Audit scores (patient interviews, chart progress notes, medication profile, etc.
  •  Upon request, provides to the patient or patient’s agent accurate and meaningful verbal and written drug and/or device information.
    • This is evidenced by QCR’s and Quarterly Audit scores (teaching checklist, patient interviews, progress notes, initial pharmacist assessment, etc.).
  • Complies with Healix Policies and procedures and recommends modifications as necessary.
    • This is evidenced by Quarterly CQI meeting minutes and audit scores.
  • Selects cost-effective and clinically appropriate drug products, methods of delivery, and delivery systems as permitted by prescribing physician.
    • This is evidence by financial reports (formulary grids) and therapeutic outcomes (care plans, progress notes).
  • Monitors therapy for each patient, perform pharmacokinetic monitoring and maintains appropriate drug use for each patient and client.
    • This is evidence by Audit scores (pharmacist assessment, care plan and progress notes, medication profile reviews and updates, and therapeutic outcomes (care plans, progress notes).Responsible to maintain required production and dispensing duties accurately as the assigned shift requires.

ABOUT US

Healix® manages physician-owned office infusion centers and pharmacies nationwide providing in-office treatment and take-home medications for optimal patient comfort and outcomes. Our on-site clinical pharmacy team consists of Clinical Pharmacy Managers and Certified Pharmacy Technicians who provide an array of pharmaceutical care services to patients within Healix-managed office infusion centers across the country. Our pharmacists and technicians are able to enjoy daytime hours within a physician office setting and have direct patient interaction in an interdisciplinary team environment.

EDUCATION and/or EXPERIENCE

  • Bachelor of Science (B.S.) in Pharmacy or Doctor of Pharmacy (Pharm.D.); two to three years of related home care or hospital experience preferred;
  • Employees must comply with all internal and external requirements for education or continuing education specific to job duties and/or licensure.
  • Strong clinical therapeutic skills and demonstration of knowledge of appropriate medication dosing
  • Proven ability to work independently in a collaborative environment, be self-directed and possess excellent organizational skills
  • Competent in the use of computers/technology

 

CERTIFICATES, LICENSES, REGISTRATIONS

  • Applicable State Board of Pharmacy License
  • ACPE Sterile Products Certification
  • CPR Certification
