Reports to: WISH Program Manager  

Employment Type: Hourly, nonexempt, 1 FTE (40 hours per week) 

Schedule: Flexible work schedule; may include some evenings and weekends 

Location: Onsite Quest Center – Multnomah  

Travel: Some local travel is required.

Our WISH (Wellness, Integrity, and Sustainable Health) Non-Opioid Pain Management Program was founded in 2015 by our Executive Director, David Eisen, and other Pain Management experts with the goal of creating a sustainable pain management program accessible to low income and underserved populations. We use a multi-dimensional approach integrates Peer support, group sessions, life skills, behavioral health, acupuncture, nutrition and more. Staying true to our roots, we emphasis group engagement that center healing in community.  

 

As our WISH Wellness Peer Mentor, you will be assisting participants in reducing barriers that hinder their activities of daily living. From intake to discharge, you’ll be working with peers by facilitating groups and individualized service planning, meeting with participants to help them meet their Service Plan goals.  Our WISH Wellness Peer Mentors work to assist our Participants to sustain lifestyle changes that support healing, and to effectively strengthen those diagnosed with chronic or life-challenging illnesses, including HIV/AIDS, mental health disorders, chemical dependency, cancer, and pain.  

 

Currently, our WISH Program is undergoing a change in participation engagement expectations. You will be helping navigate this time of change and encouraging participants to increase their participation and follow through with their service plans. This is also an exciting time, as we’re broadening the program offerings and gathering feedback from Participants to create and develop programming that aligns with those needs and wants.  

 

ESSENTIAL FUNCTIONS:  

  • Apply person-centered principles to build relationships, trust, and rapport with Participants.  

  • Assess Participant needs during intake and while in care and develop goal/action service plans accordingly.  

  • Utilize cultural competency, active listening, and motivational interviewing skills to assist Participants in reaching their treatment goals.  

  • Establish relationships with local agencies, creating connections for peer resources and referrals. 

  • Collaborate with local agencies to coordinate services.  

  • Travel with Participants to and from appointments, as needed.  

  • Assist Participants in carrying out the activities to fulfill their service plan goals and problem solve with Participants to overcome challenges and barriers.  

  • Complete chart notes, documentations, and required reports in a timely manner.  

  • Work closely with internal and local Peer Support Specialists and Certified Recovery Mentors to maximize the quality of community service.  

  • Prepare Participants for successful discharge, with appropriate skills and resources for sustainable independent self-care.  

  • Attend regular department and clinical supervision meetings.  

  • Conduct weekly 1:1 Peer Check Ins, and Lead WISH support groups and activities to engage Participants in both individual and group settings.  

  • Identify Participants barriers to engagement and/or progress in their Service Plans, work individually and programmatically to find solutions and paths forward to address these needs.  

  • Perform other duties as assigned/needed.  

Qualifications

KNOWLEDGE, SKILLS & ABILITES:  

  • Knowledgeable of co-occurring Addiction and Mental Health disorders as they relate to pain 

  • Ability to manage time and meet deadlines.  

  • Basic computer and keyboard proficiency using Microsoft Office software.  

  • Ability to be culturally conscious and work with a diverse caseload.  

  • Ability to work collaboratively as a team member, as well as independently.  

  • Ability to maintain accurate records and necessary paperwork.  

  • Must adhere to agency's non-discrimination policies.  

 

MINIMUM QUALIFICATIONS: 

  • Must hold a Certified Recovery Mentor (CRM) certification from a state-approved training program, completed prior to start date.  

  • Meets the definition of a Peer as defined by the State of Oregon Administrative Rule: a) self-identified person currently or formerly receiving mental health services; OR b) a self-identified person in recovery from a substance abuse disorder, who meet the abstinence requirements for recovering staff in alcohol and other drug treatment programs; OR c) A family member of an individual who is a current or former recipient of addictions or mental health services.  

  • Applicants in recovery from substance abuse must have a minimum of 1 year clean & sober.  

  • Demonstrated knowledge of co-occurring Addiction and Mental Health disorders as they relate to pain and be well versed in Pain Management.  

 

PREFERRED QUALIFICATIONS 

  • Exceptional applicants will have experience facilitating support groups.
