Title: Mental Health Case Manager 

Reports to: Manager of Mental Health Services

Employment: 1 FTE, Hourly, non-exempt (40 hours per week)  
Schedule: Flexible work schedule, Monday - Friday 

Location: Quest Center – Multnomah and Clackamas locations 
Travel: Local travel for outreach and partnerships required on an as needed basis 

 

ABOUT THE POSITION: 

The Mental Health Case Manager position will provide culturally specific resource navigation and support to individuals actively engaged in mental health services at Quest Center for Integrative Health. The ideal candidate is self-motivated and has a working understanding of the interconnectedness between mental health, substance use, chronic pain health support needs. Additionally, the ideal candidate has prior experience with LGBTQIA2S+ communities and is highly knowledgeable about resources available these and broader communities in the Tri-County area. This position requires a candidate with excellent organizational skills who can manage concurrent administrative projects, support an organization-wide culture of integration, and maintain a justice and trauma-informed approach in all aspects of their service work. 

ESSENTIAL FUNCTIONS: 

  • Utilize cultural responsiveness and humility as well as strengths-based/solution focused methods to research, identify, and connect participants to resources 

  • Assist participants in navigating resource brokers, systems, and care networks 

  • Apply person-centered principles to build relationships, trust, and rapport with participants 

  • Assess participant resource needs and develop goals/action plans accordingly 

  • Utilize integrative service approaches to support integrative care coordination efforts across Quest programs, and outside service organizations 

  • Assist potential participants in navigating intake processes and other treatment referrals.  

  • Remain current on treatment trends and community needs  

  • Remain current on program structures and fluency in intake processes  

  • Provide some travel support to participants making resource, referral, and treatment linkages 

  • Work closely with participant providers within Quest and without Quest 

  • Efficiently and ethically complete service and program related documentation 

  • Maintain compliance with applicable laws, rules, policies, and guidelines governing the service work and/or licensure and certification bodies 

  • Attending regular clinical supervision, program and organization-wide meetings as directed 

  • Perform other dues as assigned by manager or program coordinator 

Qualifications

MINIMUM QUALIFICATIONS: 

  • Active MHACBO certification as a Qualified Mental Health Associate (QMHA)  

  • Knowledge and experienced application of trauma-informed care and support. 

  • Prior experience with LGBTQIA2S+ communities and their nuanced service needs. 

  • Advanced knowledge and familiarity with Tri-County resources 

  • Ability to communicate and maintain relational boundaries 

  • Knowledge and understanding of acute and chronic pain. 

  • Demonstrate an understanding of systemic oppression, social justice, and equity  

  • Prior experience in the behavioral healthcare field. 

  • Advanced critical thinking skills. 

  • Ability to use and/or learn new technologies, programs, and systems in support of participants and programmatic needs. 

  • Active driver's license and ability to operate a motor vehicle for travel locally and in the tri-county area. 

  • Ability to work collaboratively as well as independently. 

  • Ability to manage time and meet deadlines. 

  • Demonstrated excellent communication, interpersonal, and organizational skills. 

PREFERRED QUALIFICATIONS: 

  • 2+ years of prior case management experience. 

  • Active MHACBO certification as a Certified Alcohol and Drug Counselor (CADC) or CADC registrant
