Summary
Under the direct supervision of the Sr. Director of Utilization Management CODACs T36 Liaison is assigned to the Court Ordered Treatment Department and is responsible for working closely with the treatment team to ensure that the team is compliant with all T36 and funder requirements.

Essential Duties and Responsibilities:
The position is responsible, but not exclusive, for the following:
  • Maintains regular contact with treatment staff to inform and be informed of members progress in order to appropriately complete and report required documentation. Information that is required to be shared with the T36 Liaison by treatment team includes but is not limited to scheduled appointments, re-engagement processes/status, court filings and Judicial Review notifications.
  • Gathers data from Court Order Treatment team and reports to applicable funder by deadlines, data includes but is not limited to:
    • Amendments, emergent and non-emergent that are filed
    • Unauthorized absences 5 or more days (so a Toll might be considered)
    • COT Dismissed due to Judicial Review Hearing
    • Enters a New COT Spans into the Provider Portal opening a COT record for the member
    • Enters a COT update each month for the member during the active COT Span
    • Updates and Closes COT Spans when appropriate
    • Sends in the spreadsheet which is the monthly COT Deliverable by the 2nd business day of the next month.
  • Ensures an annual review is scheduled and completed 45 days prior to the expiration of the court order.
  • Files requests for Tolling a members Court Order due to unauthorized absences, files status reports, and files motion when the member is re-engaged or files a motion to dismiss if the member is missing for more than 180 days
  • Files required Court filings and pleadings (amendments, requests for continuations, Change of Venues, Tolls, etc.)
  • Needs to Track BH-Inpatient Days for all members on court ordered treatment
  • Provides required court documentation to appropriate funder, including but not limited to:
    • Court Order be sure to provide the members DOB
    • Continuing Court Order
    • Change of Venue
    • Amendments (corrections only) to the Court Order i.e., Standards, name misspellings, incorrect date of births, incorrect dates, etc.)
    • Change of Supervising Agency
    • Judicial Review Dismissal
    • Request for Toll unsigned request
    • Request for Toll Signed by Court
    • Return from Toll
    • Other Early Termination or Dismissal of Order
    • Suspension of Outpatient Treatment Plan
  • Performs other related duties in accordance with agency growth and changes.

Qualifications

Education:
  • Masters or Bachelors degree in a healthcare field and no related experience OR;
  • Masters or Bachelors degree in a non-healthcare related field and 1 year relevant related experience OR;
  • Associates degree in a healthcare field and 1 year related experience OR;
  • Associates degree in a non-healthcare field and 2 years related experience OR;
  • Certified Peer Support Specialist and 2 years related experience OR;
  • High School diploma or General Education Equivalent (GED and 4 years related experience.
  • Is an Arizona Licensed Registered Nurse OR;
  • Is an Arizona Licensed Physicians Assistant who is not working as a medical practitioner;
Certifications:
  • Commission Certified Care Manager (CCCM), a plus.
Additional Requirements:
  • Valid Arizona Drivers license, proof of current insurance and willingness to use personal vehicle. Drivers license is an essential requirement for performing duties for this position.
  • Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
  • Must be able to work 12-hour shifts, evening, nights and weekends
Skills:
  • Bilingual a plus.
  • Extensive knowledge of community resources.
  • Extensive knowledge of behavioral health challenges which include knowledge of the impacts of mental illness, domestic violence, and the dynamics of addictive behavior.
  • Familiarity with various ethnic and cultural groups.
  • Intermediate to advanced computer skills using MS Office products, Word, Excel, Access, etc., importing/exporting data to/from applications.
  • Ability to communicate effectively both orally and in writing
