Summary

Provides consultation and monitoring, assessment and rehabilitation planning, guidance and counseling, skills training, job development, employment planning, specialized job placement and supported employment for the vocational rehabilitation of members with physical or mental disabilities. 


Essential Duties and Responsibilities

  • Participating as a member of the adult clinical team.
  • Engaging members by administering the Mini-Vocational Activity Profile (Mini-VAP) annually to assess member interests, strengths, and readiness for employment.
  • Introducing members to the services available through Supported Employment providers and RSA/VR.
  • Supporting members in developing skills for independent living, social integration, and recovery.
  • Care coordination with community-based rehabilitation services (e.g., education, housing, transportation).
  • Review details and expectations about the referral with members.
  • Coordinating and collaborating with Vocational Rehabilitation, Supported Employment Providers and external employment resources (e.g., RSA/VR, AZ@Work, PipelineAZ, Linkages of Arizona).
  • Supporting members with pre-employment activities.
  • Supports the development, implementation, and updating of integrated service plans, ensuring that vocational services are included, and monitors progress toward employment and recovery goals, crisis plans, and coordination with adult recovery teams.
  • Participates in Health Plan-sponsored training and interagency coordination meetings, i.e. state-wide collaborative meetings or other meetings.
  • Participates in staffing’s, Adult Recovery Team meetings or other meetings, as appropriate.
  • Collaborates with integrative health professionals, providers and other community stakeholders in order to assess employment needs and formulate employment objectives and goals.
  • Provides member advocacy, as needed, to ensure delivery of required services.
  • Maintains regular contact with treatment team staff and community stakeholders.
  • Provides community and home-based services to members.
  • Maintains member records in accordance with agency and funding source requirements.
  • Participate in provision of Desk Duty services or Counselor of the Day to provide urgent and emergent services to members on a walk-in basis.
  • Monitors member progress and provides monthly updates for deliverables and other stakeholders. 
  • Exhibits timely and accurate completion and submission of all required documentation, when appropriate and/or necessary.
  • Performs other related duties in accordance with agency growth and changes.

Qualifications

Education/Experience:
  • Masters or Bachelor’s degree in a healthcare related field, Rehabilitation counseling, vocational evaluation, vocational rehabilitation, or other relevant degree (preferred) and no related experience OR;
  • Associate’s degree in a healthcare field, Rehabilitation counseling, vocational evaluation, vocational rehabilitation, or other relevant degree (preferred) and 6 months related experience OR;
  • Certified Peer Support Specialist and 1-year related experience OR; 
  • High School diploma or General Education Equivalent (GED) and 2 years related healthcare, Rehabilitation counseling, vocational evaluation, vocational rehabilitation, or other relevant degree (preferred) experience.
Additional Requirements:
  • Valid Arizona Driver’s license, proof of current insurance and willingness to use personal vehicle.  Driver’s license is an essential requirement for performing duties for this position.
  • Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
  • Must be able to work weekends and holidays as needed. 
  • Must be able to travel to members’ home environments, as needed.
Skills:
  • Follow SAMHSA’s Practice Principles of Supported Employment
  • Bilingual a plus
  • Extensive knowledge of community resources.
