General Summary

This position provides screening, assessment, treatment planning, crisis intervention, case management and supports coordination services to persons with mental health disabilities. 

 

This position reports to: Mental Health Supervisor.

 

Essential Functions 

  1. Interviews prospective persons served regarding eligibility for services and provides comprehensive biopsychosocial assessments to determine appropriate benefits plan. 
  2. Schedules and conducts person centered planning activities, including persons served, family members of persons served, and other individuals and agencies as requested by the person served. 
  3. Develops individual plans of service and enrolls and authorizes these plans with the managed care entity. Amends and updates these plans as indicated. 
  4. Provides ongoing case management and supports coordination services as required. This includes assisting the person served and/or family members and guardians with planning and accessing needed services and supports in the areas of mental health treatment, entitlements, health, housing, education, employment, and social/recreational needs. 
  5. Participates in triage services, providing assessment, crisis intervention, and referral services. Develops and maintains effective working relationships with other community agencies. Participates in collaborative community planning groups and provides community education activities as assigned. 
  6. Arranges and/or provides transportation services to needed services for assigned caseload as required. 
  7. Participates in guardianship hearings and other court-related matters for assigned caseload as required. 
  8. Completes clinical documentation in a timely manner, consistent with agency, state, and federal requirements. This includes maintenance of both written case files and electronic medical records for assigned caseload.

Other Functions 

  1. Compiles and prepares reports as assigned.
  2. Participates in training and continuing education opportunities as assigned.
  3. Other duties as assigned by the Mental Health Supervisor.

Qualifications

Employment Qualifications

 

Education: Bachelor’s degree in counseling, psychology, social work, or mental healthrelated field. 

 

Experience: 

  1. Six months to one year of experience in human services, preferably working with persons with mental health-related disabilities. 
  2. Lived experience with behavioral health issues preferred. 

 

Other Requirements: 

  1. Good organizational skills 
  2. Ability to write and speak effectively. 
  3. Possession of a valid Michigan’s driver’s license.
