General Summary

This position performs a variety of direct service functions for persons served of mental health services, including services for adults with developmental disabilities, adults with serious mental illnesses, and adults with substance abuse disorders.

 

This position reports to: Program Supervisor

 

Essential Functions

  1. Directly supervises persons served during service activities (both on site and in community-based settings).
  2. Carries out daily program plans as prescribed through the individual plan of service of the person served and as assigned by the supervisor.
  3. Provides transportation services when necessary.
  4. Assists persons served with the administration of medication as prescribed.
  5. Provides training and education services to persons served.
  6. Assists with the preparation of program planning, organizing and assembling materials and proper cleanup and storage of materials and supplies.
  7. Participates in person centered planning-related meetings as requested.
  8. Documents the progress of consumers through the completion of case record.
  9. Completes other agency documentation as required.

Other Functions

  1. Participates in training and continuing education opportunities as assigned. 
  2. Other duties as assigned by the Mental Health Supervisor.

Qualifications

Employment Qualifications

 

Education: High school graduation

 

Experience:  

  1. Entry-level position.
  2. Prefer experience working with adults with mental health disabilities (developmental disabilities and/or serious mental illnesses)

 

Other Requirements:

  1. A current or previous primary person served of publicly funded mental health services.
  2. Ability to work with persons served and providers of mental health services. 
  3. Ability to write and speak effectively.
  4. Possession of a valid Michigan driver's license or ability to travel to and access assigned service sites.
  5. Possession of certification from the Michigan Department Health & Human Services (MDHHS) as a “Certified Peer Support Specialist”.

Working Conditions:

  1. Works in office settings and in community-based service sites throughout the county.
  2. Required to travel within the county.
  3. Delivers care to persons served with a range of physical and mental healthrelated disabilities.

Exposure to communicable diseases, blood, and other potentially infectious bodily fluids and materials.
