Position Summary
The Licensed Counselor - Assertive Community Treatment (ACT) is a vital member of our multidisciplinary team, providing in-person, community-based mental health care to individuals with severe and persistent mental illness. This position focuses on client engagement, clinical assessment, treatment planning, and crisis support, with the goal of helping clients remain safely in the community while improving their quality of life.

Essential Responsibilities
- Conduct comprehensive evaluations of clients’ emotional, social, and psychiatric histories
- Develop, implement, and monitor individualized treatment and safety plans
- Coordinate care across internal teams, hospitals, first responders, and community partners
- Provide direct clinical interventions and crisis response in community settings
- Facilitate hospital admissions when necessary and support continuity of care
- Maintain regular communication with clients, families, and support networks
- Document all services in the electronic health record system in a timely and professional manner
- Serve as a liaison with crisis hotlines, emergency departments, and other referral sources
Qualifications
- Education/Licensure: Active Colorado licensure as LCSW, LPC, or LMFT (required)
- Experience: Experience working with clients in crisis and/or severe mental illness (preferred)
- Knowledge/Skills:
- Strong understanding of clinical mental health practices
- Excellent verbal, written, and interpersonal communication
- Skilled in complex problem-solving, ethical conduct, and service coordination
- Ability to work independently in a variety of community environments
- Valid Colorado driver's license with a clean driving record
