Requirements:Â
- Current licensure as a social worker by the State of Ohio Counselor and Social Work Board, or as a Registered Nurse with the Ohio Board of Nursing. Â
- Two years of experience in home care, geriatrics, home health care, medical social work, or other substitute as approved by the Ohio Department of Aging. Â
- Basic knowledge of major chronic illnesses, disease processes, normal aging, and basic knowledge of medical terminology. Â
- Excellent verbal and written interviewing and assessment skills.Â
- Knowledge of local resources for older adults. Â
- Ability to communicate in a collaborative, effective manner with others and maintain good working relationships.Â
- Intermediate computer skills including Microsoft Office applications. Â
- Ability to complete job duties by telecommunication from home and field.Â
- Ability to record information in an accurate, timely manner and meet each program’s time frames.Â
- Ability to use and will be trained in the Ohio Department of Aging and Ohio Department of Medicaid documentation systems.Â
- Ability to use and transport a laptop computer and case, therefore must be able to lift/carry a minimum of twenty (20) pounds.Â
- Ability to ambulate enough to access private homes/locations which may not be readily accessible.Â
- Must have a car available during working hours, as well as the ability to legally drive a car. Â
