Givens Aldersgate is looking for an experienced MDS Coordinator who will be responsible for facilitating/coordinating the Care Plan process. In this role you will accurately assess, implement, and document resident care. You will work in a collaborative manner, advise, and teach other personnel involved in resident care.
What you’ll do:
- Act as team leader for the Interdisciplinary Care Plan Team;
- Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations and guidelines that govern the resident assessment;
- Complete all assessment components which may include assessments such as fall assessment, cognition assessment, wandering assessments, self-administration of medication assessment, ROM/contracture/functional mobility assessments, pressure ulcer assessment and bowel & bladder assessment or train a specific designee to do these assessments and evaluate the assessor annually that they continue to do the assessments accurately;
- Make Medicare eligibility determinations;
- Complete all Discharge and Re-entry assessments as required by regulation;
- Maintain an ongoing quality assurance program for the resident assessment/ care planning process and the restorative care program;
- Coordinate the completion of RAP’s for each triggered item per current regulations;
- Complete the Care Plan that meets current regulatory standards for each resident;
- Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor.
What you’ll need:
- Valid RN license
- Experience as an MDS in short-term care
- Knowledge of Resident Assessment Instrument
- Strong attention to detail
- Strong clinical skills
- Understanding of regulations, policies and procedures
Compensation is $85,000 - $95,000 per year, plus our comprehensive benefits package!
