What We’re Looking For: 

This role supports the Director of Off Platform in growing and optimizing the Soluna Affiliate Network by managing the full cycle of partner relationships and enhancing care coordination processes. It requires strong problem-solving and project management skills, with responsibilities including cross-departmental collaboration, vendor management, and ensuring partners are set up on the Findhelp platform. The goal is to foster long-term engagement and smooth transitions of care for Soluna Service Users.

How You’ll Make an Impact: 

Affiliate Network Development and Expansion

  • Lead the implementation, maintenance, and continuous improvement of the Soluna Affiliate Network, ensuring alignment with organizational goals and community impact.
  • Oversee the end-to-end process of Affiliate Network growth-including project tracking, partner agreements, technical setup, onboarding, and ongoing relationship management.
  • Develop and lead Affiliate Network-wide meetings to maintain engagement with partners and provide updates regarding Soluna development.
  • Maintain the Soluna Findhelp resource database. Manage and maintain Soluna Favorites and Featured program updates with new and established Partners. Findhelp subject matter expert for using and configuring Findhelp. Provide hands-on, technical assistance to new and established AN Partners. 
  • Work cross-functionally with the Integration and Partnerships teams to receive and provide relationship referrals for respective partnership activities. 

Stakeholder Management:

  • Develop and sustain strong relationships with internal and external stakeholders, including external provider groups, community agencies, and other service organizations that provide resources for the population served by Soluna.
  • Proactively manage stakeholder expectations and resolve potential conflicts to foster long-term collaboration and success.
  • Collaborate with the Off Platform Operational Manager to support care navigation efforts, particularly in relation to care coordination with Affiliate Network Partners and other agencies via the Findhelp Platform.

 Service Developments:

  • Work closely with cross-functional teams-including Service Delivery, Clinical, Integration & Partnerships, Product, Engineering, and Marketing, to continuously evolve and improve our services, ensuring they meet the diverse needs of our communities.
  • Recommend enhancements and adjustments to care coordinator and Affiliate Network processes. Lead and support improvements by reviewing Care Navigation data and identifying opportunities to address gaps and optimize program success. 

Analytics, Reporting & Insights:

  • Track and report on Affiliate Network development and partnership performance. Provide recommendations for opportunities for improvement accordingly.
  • Identify gaps in service coverage of signed network agencies and/or geographic regions.
  • Track key metrics and KPIs to measure affiliate partner success and satisfaction. 

Qualifications

What You’ll Bring: 

Education and Experience Requirements:

  • 3–5+ years of experience managing stakeholder, partner, or vendor relationships, with a proven ability to build trust and drive collaboration
  • 3–5 years of experience in the healthcare sector or working with social determinants of health, with a strong understanding of community impact
  • Solid understanding of the U.S. healthcare ecosystem, including payers, providers, and community-based organizations
  • Excellent written and verbal communication skills; able to tailor messaging for diverse audiences
  • Strong project management capabilities with experience leading cross-functional initiatives from planning through execution
  • Demonstrated ability to deliver operational outcomes, improve processes, and maintain attention to detail in a fast-paced environment
  • Skilled in data capture, analysis, and translating insights into action
  • Highly adaptable and flexible, with a growth mindset and comfort navigating change
  • Effective at collaboration and team engagement, contributing positively to a mission-driven culture
  • Solution-oriented problem solver with a bias toward action and accountability
  • Strong time management and organizational skills; able to manage multiple priorities and meet deadlines

Compensation: The base salary for this role Approximately $70,000 - $95,000. We’re committed to transparency and value our candidates time, which is why we share salary ranges in all states—regardless of local requirements. Final compensation will be based on a variety of factors, including your education, experience, skills, and overall alignment with the role.
