The MDS Nurse Manager is responsible for completing and transmitting all MDS/RAPS/CAAs/Care Plans, per state and federal mandated guidelines for Skilled Nursing. This role will also provide nursing care for Skilled Nursing, Assisted Living and Memory Enhancement Members, as well as scheduling and conducting Care Plan Meetings with Members and families for Skilled Nursing. Also responsible for creating and updating Care Plans for Assisted Living and Memory Enhancement Members.

Reporting Relationship:
- Reports directly to the Director of Nursing.
Principle Accountabilities:
- Participates in the admission process of prospective members in terms of their nursing needs and appropriate care levels.
- Works in collaboration with the Interdisciplinary Team (IDT) to assess the needs of the Members.
- Coordinates and manages the weekly Medicare Meeting with the IDT.
- Complies with federal and state regulations regarding coordination and completion of the RAI/MDS process.
- Maintains current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicates changes in regulation.
- Monitors Medicare assessment schedules (time frames and due dates) and nursing documentation to ensure accuracy and timely submission.
- Assists Director of Nursing in training and development for nursing staff.
- Assists Director of Nursing in monitoring and tracking clinical programs.
- Reviews Service Plans for Assisted Living and Memory Enhancement at least monthly and makes changes/updates as necessary.
- Participates in quarterly QAPI meetings.
- Observes all facility safety policies and procedures.
- Identifies resident problems and emergency situations and intimates emergency care as necessary.
- Follows, supervises and evaluates the implementation of the Resident Rights protocol.Â
- Reports any possible cases of Resident Abuse immediately to the Director of Nursing and the Administrator.
- Provides respect, privacy and dignity to all Prairieview residents
- May cover for other nursing staff on Skilled Nursing, Assisted Living and/or Memory Enhancement in case of absence or illness and providing more direct member care.
- Assists to interpret policies and procedures to nursing assistants, other related departments, physicians, residents and family members.
- Follows established standards of nursing practice and implements facility policies and procedures.
- Completes Infection Preventionist Training and acts as the facility’s designated Infection Preventionist.
- Responsible for overseeing and maintaining the facility Antibiotic Stewardship Program.
- Observes and supervises all facility infection control procedures.
- Participates in quarterly QAA meetings.
- Observes all facility safety policies and procedures.
- Identifies safety hazards and initiates corrective action immediately.
- Identifies resident problems and emergency situations and intimates emergency care as necessary.
- Follows, supervises and evaluates implementation of the Resident Right’s protocol.
- Reports any possible cases of Resident Abuse immediately to the Director of Nursing and the Administrator.
- Follows Abuse allegation protocol as directed by Administrator.
- Provides respect, privacy and dignity to all Prairieview residents; does not enter any resident room inappropriately.
- Listens to resident, family and physician concerns and initiates corrective actions and communicates concerns to Director of Nursing.
- Consistently works cooperatively with residents, other nursing staff, physicians, family members, consultant personnel, community agencies and ancillary service providers.
- Participates in facility education programs.
- Attends staff meetings, interdisciplinary team meetings, in service classes and committee meetings as assigned.
- Assists with orientation of new residents to facility.
- Investigates all door alarms immediately, affirming the cause of the alarm prior to clearing and resetting of the alarm.
- Responds in a timely manner to the emergency call system and portable paging system.
- Comes to work clean and neat, consistently presenting an appropriate professional appearance.
- Performs all other duties as assigned.

ADMINISTRATION
- Completes Incident Reports accurately and timely.
- Completes Concern Reports accurately and timely.
- Completes all documentation regarding medication administration and treatments accurately and timely.
- Documents any resident change in condition per policy.
- Completes all necessary documentation of an allegation of resident abuse, and communicates the allegation immediately to the Director of Nursing and Administrator.
Knowledge, Skills, & Abilities:
- Experience working in acute and/or long-term care environment preferred.
- Licensed in the state of Illinois as a Registered Nurse.
- Must possess current RAC-CT Certification for MDS.
- Must possess, or be able to obtain current CPR certification.
- Must have an adequate understanding of English and be able to speak and write it well.
- Must be able to work independently, as well as a team member, with a positive, pro-active attitude.
- Is flexible and adapts to change and new tasks.
- Follows all policies and procedures of Prairieview at The Garlands.
- Attends staff meetings, educational programs and resident care plan meetings.
- Must be familiar with the resident rights, the definition of Resident Abuse, and the reporting protocol of possible Resident Abuse.
- Must be a flexible, highly customer service and people oriented individual who demonstrates compassion, patience and understanding, and is willing to do whatever it takes to provide the highest level of service.
