GENERAL SUMMARY
Â
This is a full-time, exempt position. The ACC Care Center Social Services Manager directs programs in response to the social and emotional needs of the residents; assesses, develops, and implements care plans for social service needs of residents; works with the Admissions Coordinator on admissions to and discharges from the facility; maintains policies and procedures for the social services of the residents, and participates in special projects as assigned. Â The Social Services Manager reports to the Administrator and is responsible for supervising the Social Services Assistant.Â
ESSENTIAL JOB FUNCTIONS:

- Identifies psychosocial needs of residents during admission and upon staff or physician referral; makes referrals to community agencies.
- Develops Social Services plan of care and updates the plan as changes occur for each resident throughout their stay.
- Participates in resident care conferences; manages written notification to resident and/or families of conference times.
- Completes all social service assessments, MDS entries, and care plans within the required time frames and actively participates in the Interdisciplinary team process.
- Coordinates, helps facilitate room changes for residents including notification to residents, family members, roommates, other departments.
- Develops discharge plans for short-term residents according to wishes and physician’s orders; keeps Interdisciplinary team informed of resident discharge plans.
- Advocates for resident rights; works with other departments to meet resident needs.
- Oversees dental, podiatry, vision and hearing appointment coordination in facility; ensures consents are in place and coordinates payment information.
- Oversees transportation arrangements for resident appointments; coordinates with family/responsible party to escort resident; coordinates with nursing department for escort.
- Coordinates the Theft and Loss program; ensures that all reports are investigated and resolved; keeps Administrator informed of issues; reports Theft and Loss outcomes to the QA&A Committee quarterly.
- Responds to requests or complaints by residents or family members; maintains Facility Complaint Log and keeps Administrator informed of issues.
- Maintains current references of community resources that may be required and provides information to residents and family members as needed; responsible for coordinating family social and educational events.
- Provides facility tours.
- Arranges committee meetings; facilitates Family Council meetings.
- Arranges access to Paratransit and other assistance for residents.
- Assists in obtaining legal assistance for residents.Â
- Advises Director of Nursing per situations which may have adverse effect upon patient care.
- Maintains confidentiality and full HIPAA compliance.
- Assists clients in a timely manner; takes initiative to resolve resident concerns.
- Practices workplace safety at all times and reports repair needs to maintenance.
