This position is a cross organization, collaborative, systems focused position to support the assigned department(s) in ensuring compliance with person-centered planning, stakeholder feedback and personal outcomes of those served. The Support Coordination Manager will be responsible for working closely with internal PCP Facilitators, Coordinators of Community Services and the Developmental Disabilities Administration.

This position requires a high school diploma or GED with at least three years of professional experience providing person centered planning for people with disabilities, though a bachelor’s in social work or a related field is a plus. Candidates must also possess a valid driver’s license with no egregious records and complete all required Fello and Developmental Disabilities Administration (DDA) trainings, becoming trained as a Certified Medication Technician.
