Hinds Hospice, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a LVN Intake Coordinator to join our team.Â
The LVN Intake Coordinator is responsible for taking all hospice information calls regarding the intake of new referrals to facilitate an evaluation for hospice service. The LVN manages all patient intake functions throughout the entire intake process and provides Hospice evaluations as directed.
