Description

 

Hinds Hospice, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Full Time RN Liaison to join our team. 

 

The RN Liaison is dedicated to serving the hospital and its staff and those patients eligible to access their Medicare Care Hospice benefit.  The Liaison will conduct clinical evaluations in collaboration with local hospitals, facilities and healthcare agencies, to determine hospice eligibility and perform admissions of patient referrals for both Inpatient and Outpatient Services under the direct supervision of the Director of Inpatient Hospice Services.

 

  • Provide the hospital staff in-services and education on hospice and palliative care related matters.  Increase awareness around the edibility related to hospice.
  • Become involved in the hospital’s committee structure as allowed that relate to strengthening end-of-life care.  Seek opportunities to become part of the hospital’s fabric and culture.
  • Collaborate with hospital staff to assess and determine hospice eligibility of potential patients in need. Exhibits a high level of customer service and utilizes effective communication skills to improve service focus to support referral sources.
  • Supports and collaborates with Director of Inpatient Hospice Services to meet census goals for the Inpatient Care Unit.
  • Respond promptly for evaluations or informational meetings with referral sources, patients/families.
  • Perform Hospice admissions for Hinds Hospice.
  • Manages a patient and family caseload within the hospital for those cases considering hospice care.
  • Work with Hinds HCC to develop and implement plans to better serve the hospital’s end-of-life care needs. Create specific accounts within the hospital that could benefit from focused hospice education and training.

Qualifications

 

  • Compensation $60 - $62 hourly

Qualifications include but are not limited to:

 

  • Completion of a professional nursing program accredited by the Board of Registered Nursing with completion of a baccalaureate program in nursing preferred.
  • Five to seven years of nursing experience, with 3 years in a hospice. Experience as an Emergency or ICU nurse preferred.
  • Demonstrates the ability to function independently with strong clinical skills and critical thinking abilities as relates to patient care and staff responsibilities.
  • Exhibits sensitivity to cultural differences and has knowledge of current trends in home health care, federal and state regulation, state licensing requirements and other standards related to hospice care.
  • Current RN Licensure, CPR, recent physical and TB, Tdap, MMR, Varicella, HepB immunizations.
  • Must have a valid driver’s license and a willingness to use personal vehicle in the course of employment.
  • Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36-month period).
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35lbs)
