Role:
The Early Childhood Case Manager plays a vital role in supporting families and strengthening community connections. This role combines case management, community engagement, and resource coordination to ensure families receive individualized support that promotes wellness, academic success, and community development. Reporting to the Village Wellness Center Director or their designated person, the Case Manager delivers culturally responsive services tailored to the needs of families served by the Village Wellness Center. This position requires a detailed understanding of targeted case management and documentation. Applicants having an understanding of Early Head Start, Head Start, and DFSS policies and rules is preferred.

Essential Duties and Responsibilities:

Case Management: Provide personalized case management services to families by conducting structured family interviews and utilizing assessment tools to identify strengths, needs, and risk factors. Use this information to develop individualized service plans, monitor progress, and coordinate referrals to appropriate resources that support family stability and well-being.

Family Support & Engagement: Offer direct support to families through one-on-one meetings and phone consultations, building strong relationships to foster engagement and trust.Â

Community Resource Coordination: Collaborate with local service providers to connect families to resources that promote wellness and academic success, ensuring that families are informed about available services.Â

Workshops & Orientations: Support the VWC Clinical Director and Case Management Supervisor in organizing and facilitating orientations, workshops, and family education events to promote relevant family and community topics.Â

Documentation & Reporting: Maintain accurate and confidential family records in accordance with VWC standards and participate in regular audits to ensure service quality and compliance.
