SUPERVISOR: Vice President, Sonoma County

STATUS: Full-time, Exempt

COMPENSATION: $87,360/Annually

We serve the Bay Area community through low-barrier programming and systems leadership – meeting each person where they are by offering support and resources that address immediate and long-term housing needs. With 45 years of experience, we view housing as a fundamental human right and understand that solving homelessness requires a collaborative effort.

 

PROGRAM OVERVIEW

A By-Name List (BNL) is a comprehensive list of every person in a community experiencing homelessness, updated in real time. Using information collected and shared with their consent, each person on the list has a file that includes their name, homeless history, health, and housing needs. The Petaluma and Sonoma Valley BNLs are the vital tools that are used in case conferencing meetings held weekly. Sonoma County has several sub-regional BNLs and one comprehensive, county-wide BNL. The By-Name-List Coordinator is responsible for maintaining the Petaluma and Sonoma Valley BNLs, facilitating all related activities in these communities, and coordinating with wider regional efforts to end homelessness.  

 

REPORTING RELATIONSHIPS

The position reports to the Vice President, Sonoma County.

 

PRIMARY RESPONSIBILITIES

By-Name-List Leadership

  • Convenes local service providers on a regular basis
  • Creates agendas, taking meeting minutes, and sending notes and follow up to attendees
  • Maintains By-Name-Lists of people currently experiencing homelessness in South County and Sonoma Valley, including a regular analysis of inflow and outflow and updating all relevant client information from both the Homeless Management Information System and report outs from local partners
  • Develops housing and engagement strategies for people on the list in coordination with local service providers
  • Drives actions and next steps between meetings to ensure clients are receiving the support that they need
  • Provides quarterly data visualizations detailing the overall size of the By-Name-List, as well as key metrics including but not limited to shelter status, Coordinated Entry status, and housing resources identified and utilized
  • Presents data and progress updates to stakeholders, including the Petaluma and Sonoma City Councils
  • Continually improves case conferencing and By-Name-List policies, procedures, and practices by actively researching and connecting with other efforts throughout the region and country
  • Codifies meeting and By-Name-List policies and procedures within a group charter
  • Provides individual technical assistance as needed or requested to local providers’ staff on By-Name-List policies, procedures, and practices
  • Works with regional partners including the County, Coordinated Entry, other government agencies, and other social service providers to ensure regional care coordination; this includes reconciliation of regional lists with sub-regional lists
  • Develops new partnerships and workflows to connect available resources to all providers participating in the By-Name-List process
  • Attends regional care coordination meetings including countywide Coordinated Entry case conferencing and countywide subregional care coordination efforts, and coordinates progress towards housing for Petaluma participants
  • Represents and communicating the general needs, interests, and desires of people experiencing homelessness in the community

 

General

  • Attend all job-related meetings, including staff meetings, agency-wide meetings and regional meetings
  • Represents the agency in public speaking events as requested
  • Participates in opportunities for learning and skill maintenance/development, including internal and external training and workshops
  • Complies with and upholds all local data sharing and privacy agreements
  • Follows all agency policies and procedures
  • Assists with other duties assigned

Qualifications

QUALIFICATIONS (YOU HAVE)

Knowledge and Experience, General

  • People with lived experience encouraged to apply
  • 1 year experience in human services
  • Demonstrated experience facilitating groups or meetings
  • Demonstrated experience in basic data analysis and visualization
  • Bachelor of Arts in Social Work (BASW) degree or related human services field preferred, not required
  • Knowledge of homelessness and/or housing services preferred
  • Proficient in Microsoft Word, Outlook, and Excel
  • Valid CA driver’s license and insurance, and reliable transportation to use on the job
