YSS is seeking an organized, dedicated, and compassionate Ember Clinic Nurse - LPN to join our Ember Recovery Clinic Operations team in Cambridge. In this role, you will be part of a multidisciplinary team to to ensure health, safety, and well-being of clients while addressing their physical and mental health needs. The LPN will assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services in our residential program. The LPN also provides support with medication management and service/treatment referrals. Other responsibilities include:
- Provide Therapeutic Care and Nursing Interventions.
- Use available data, assessments, evaluations, and observations to identify health needs and develop care plans.
- Monitor high-risk patient cases.
- Develop symptom and crisis response.
- Prepare and administer injections, including vaccinations.
- Educate clients on preventing and managing illnesses and chronic conditions.
- Manage medication distribution and integration into treatment plans.
- Evaluate and document psychotropic medication effectiveness and side effects.
- Monitor refrigerated medications in compliance with regulations.
- Communicate with pharmacies for medication refills.
- Provide training and support on medication management.
- Professionally and effectively communicate with patients, families, medical professionals, and staff.
- Facilitate and support collaboration and communication between physical health, behavioral health, and prescribing providers.
- Complete patient documentation as per YSS and regulatory agency policy.
- Assist with quality assurance and utilization reviews.
- Schedule client appointments.
- Attend regular meetings and trainings.
- Participate in medical on-call system, including nights weekends, and holidays
