Summary

We are looking for a skilled and compassionate Physical Therapist (PT) to join our Home Health team. The ideal candidate will provide specialized rehabilitation services to patients in their homes, focusing on improving mobility, strength, and overall quality of life. This role requires effective communication with the home health prescribing physician, nurse practitioner, or physician assistant to ensure coordinated and comprehensive patient care.

Key Responsibilities

  • Patient Evaluation: Conduct thorough initial assessments of patients' physical abilities, functional limitations, and rehabilitation needs. Utilize standardized tests and measures to establish baseline data and identify goals.
  • Care Planning: Develop personalized treatment plans based on assessment findings, patient goals, and physician recommendations. Collaborate with the healthcare team to ensure integrated care across disciplines.
  • Therapeutic Interventions: Implement individualized physical therapy interventions, including exercises, manual therapy, modalities, and patient education. Adjust treatment plans as needed based on patient progress and feedback.
  • Communication: Maintain ongoing communication with the prescribing physician, nurse practitioner, or physician assistant regarding patient status, treatment efficacy, and any changes in patient condition. Document all interactions and updates in the patient’s electronic health record (EHR).
  • Patient Education: Educate patients and their families about rehabilitation processes, home exercise programs, and strategies to enhance safety and mobility. Provide resources for continued care and wellness.
  • Functional Training: Assist patients in achieving independence in daily activities through functional training and adaptive techniques. Focus on improving mobility, balance, strength, and coordination.
  • Monitoring Progress: Regularly assess patient progress toward treatment goals and modify interventions accordingly. Schedule follow-up visits to evaluate outcomes and ensure continuity of care.
  • Interdisciplinary Collaboration: Work closely with other members of the healthcare team, including occupational therapists, speech therapists, and home health aides, to provide holistic care. Participate in team meetings to discuss patient cases and share insights.
  • Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with regulatory standards and organizational policies.
  • Compliance and Safety: Adhere to all safety protocols and regulatory guidelines while providing care in patients' homes. Ensure that equipment used during therapy sessions is safe and appropriately maintained.

 

Qualifications

Qualifications

  • Education: Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) from an accredited program.
  • Licensure: Valid Physical Therapist (PT) license in [State].
  • Experience: Minimum of [X] years of clinical experience, preferably in home health, geriatrics, or rehabilitation settings.
  • Certifications: Basic Life Support (BLS) certification; additional certifications in specialized areas of physical therapy preferred.

Working Conditions
