Description

In an environment of continuous quality improvement, the Admissions Coordinator is responsible for providing pre-admission assessment and marketing activities for Memorial Health System’s swing bed program.  Identifies patients who meet program admission criteria and facilitation of admissions to the program. This role requires travel to various locations where needed. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.

 

Job Functions:

  1. Educate referral sources, family members, and external payers regarding appropriate utilization of swing bed services.
  2. Preadmission screening conducted in person or through review of the patient’s referring hospital medical records
  3. Facilitate admission of appropriate patients, including coordination with referring and admitting physicians, nurses, therapists, discharge planners, third party payers, and family members.
  4. Provide recommendations for appropriate care for potential patients who are denied admission to the unit.
  5. Coordinate pre-certification with third party payers.
  6. Develop relationships with referral sources.
  7. Participate in marketing activities.
  8. Prepare and submit activity reports.
  9. Attend all mandatory in-services within the time frame offered.  Participate in quality improvement teams.
  10. Maintain positive attitude and commitment to personal service with patients, visitors, and co-workers as demonstrated by routine helpfulness, initiative in assisting others and demonstrates respect, dignity, and caring. 
  11. Promote effective working relationships through mutual respect and honest with rehab team members.  Remain flexible and adapts to change in a professional manner.  Resolves conflict by using appropriate interpersonal skills.
  12. Assumes all other duties and responsibilities as necessary.

Qualifications

Minimum Education/Experience Required:

  • Associate’s degree in Nursing required. Bachelor’s degree preferred.
  • Current licensure in the State of Ohio as a Registered Nurse required.
  • Previous care management or admission coordination experience preferred.
  • Must have a valid driver’s license and remain insurable by hospital’s agent.
  • Required to travel between various locations throughout the health system as assigned.

Compensation Details:  Education, experience, and tenure may be considered along with internal equity when job offers are extended.
