Position Summary:  

Provides high-quality provision of comprehensive primary and preventive psychiatric health care services to patients with complex addiction and substance related health conditions throughout our clinical sites.  Works as a core member of the Collaborative Care Team which involves the patient’s primary care provider and other mental health, addiction, or substance abuse providers, as appropriate, to provide comprehensive patient centered care including psychiatric evaluations, psychotherapy, medication evaluation, and/or medication management as part of an integrated team-based practice within Family Hospital’s network.  Works independently and interdependently with other clinic departments and providers to ensure integrated care for patient centered outcomes.  

Essential Functions: 

 

Role Specific 

  • Provides, supports, and closely coordinates mental health care with the patient’s primary care provider and, when appropriate, other mental health providers. 

  • Screens and gathers information from patients regarding common psychiatric, mental health and substance abuse disorders and screens for physical, developmental, emotional genetic, neurological, environmental, or other social or cultural risk factors. 

  • Supports psychotropic medication management prescribed by primary care providers to address treatment adherence, side effects and other complications, and effectiveness of treatment. 

  • Assists other physicians and providers in revision and implementation of practice principles, clinical protocols and health care plans as needed. 

  • Performs psychiatric examinations, evaluations, assessments, diagnoses, and treatment. 

  • Provides psychiatric medical consultation to providers and patient care teams. 

  • Documents patient histories, progress notes, interventions, and orders in the electronic health record. 

  • Works collaboratively with a multidisciplinary team. 

  • Establishes and maintains rapport with community agencies. 

  • Documents patient encounters, patient progress, treatment, and follow-up care in the electronic medical record system within the scope of practice. 

  • Facilitates referrals for clinically indicated services outside Family Hospital (mental health specialty care, substance abuse treatment, housing assistance, etc.).

  • Applies the principles and values of customer service and continuous quality improvement while performing day to day activities of the position. 

  • Identifies barriers to effective communication based on psychosocial, literacy, financial, and cultural considerations. 

  • Demonstrates respect, equity, and empathy in actions and interactions with culturally diverse and vulnerable. 

  • Partners with the healthcare consumer to enhance the care provided on the unit. 

  • Demonstrates caring attitudes and behaviors to develop therapeutic relationships. 

Compliance 

  • Safeguards all patients’ health information whether written, verbal, and electronic with privacy and confidentiality.  Follow HIPAA Privacy and Security policies and procedures. 

  • Maintains confidentiality of patients and staff. 

Quality 
Participates in Quality Improvement activities to ensure compliance with Family Hospital Policies, including peer review of other appropriate health providers. 

  • Uses evidence-based interventions and strategies to achieve optimal outcomes. 

  • Participate in unit-based Quality improvement, evidence-based practice and research activities. 

  • Use current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues.

  • Consistently utilizing provided resources to learn and reference facility policy. 

  • Seeks out learning opportunities and continuing education. 

  • Seeks experiences that reflect current practice in order to maintain skills and competence in clinical practice or role performance.

  • Completes required hospital training annually and any additional training as requested or required by nursing leadership.

  • Maintains current licensure/certification for position, if applicable.

  • Consistently follows departmental and hospital Health, Safety, Security, Hazardous Materials policies and procedures. 

  • HIPAA: Conduct job responsibilities in accordance with HIPAA privacy laws, follow hospital policy in provision of patient confidentiality. Able to identify patient confidentiality issues and reports to proper hospital personnel immediately. 

  • Compliance: Conducts job responsibilities in accordance with standards set forth in FHMC Code of Conduct, FHMC policy and procedures, applicable federal and state laws, and applicable standards. 

  • Employee must maintain a courteous and respectful attitude toward fellow employees, staff, contractors, vendors and the public at all times. Employee must avoid loud, profane, or unprofessional language at all times during the performance of duties. It is immediate grounds for termination if Employee engages in misconduct or is incompetent or negligent in the proper performance of duties or is disorderly, dishonest, intoxicated, or discourteous

 

 

Qualifications

Qualifications: 

 

Education: Masters or Doctoral degree in Nursing with a Psychiatric certification 

 

Licenses/Certification: Required: APRN License, PMHNP-BC, BLS 

Preferred: Dual certification: FNP-BC or AGACNP-BC 

                      

Experience:   Preferred: Two (2) to three (3) years’ experience in a clinical setting.
