Overview:
Ampla Health is seeking a compassionate and organized Lead Case Manager to support our Enhanced Care Management (ECM) and Transitions of Care program. This role connects patients with clinical and social support services, coordinates post-discharge care, and ensures seamless transitions between care settings.
Key Responsibilities:

Conduct patient outreach and enroll eligible individuals into the CalAIM ECM program.

Complete comprehensive assessments and develop personalized care plans.

Coordinate care across departments and with external partners.

Support patients with medication adherence, appointment scheduling, and service navigation.

Monitor care plan progress, update records, and track program metrics.

Participate in quality improvement initiatives and staff training.
Qualifications:

High school diploma required; CMA certification preferred.

Valid CA driver’s license and proof of insurance.
-
CPR certified.
-
Excellent communication, organization, and interpersonal skills.

Experience working with diverse populations and care coordination.
