Ampla Health is seeking a Licensed Vocational Nurse (LVN) to lead and supervise staff in our CalAIM Enhanced Care Management program. This role ensures smooth day-to-day operations, guides Lead Case Managers, and supports care coordination for complex patient needs.
Key Responsibilities:

Supervise and support CalAIM staff and case managers

Monitor program metrics and ensure KPIs are met

Oversee patient outreach, care planning, and documentation

Provide staff training, coaching, and performance reviews

Maintain compliance with billing and program guidelines

Collaborate across departments to improve patient outcomes

Perform nursing duties as needed
Requirements:

Active CA LVN license and current CPR certification

5+ years healthcare experience; 1+ year in supervision preferred

Strong EHR and care coordination skills

Knowledge of CalAIM program and community resources
