Description

Ampla Health is seeking an Outreach and Enrollment Specialist to connect patients with health coverage, benefits programs, and community resources. This role provides education, enrollment assistance, and outreach to improve access to healthcare and supportive services.

Key Responsibilities:

  • Assist patients with health insurance and benefit applications (Covered CA, Medi-Cal, CalFresh, Medicare, Rx Assistance, etc.)

  • Provide patient education on Ampla Health services and available programs

  • Coordinate patient appointments, transportation, and follow-up care

  • Refer patients to community resources addressing social determinants of health (housing, food, etc.)

  • Serve as a liaison between Ampla Health and community organizations

  • Attend and represent Ampla Health at outreach events, presentations, and health fairs

  • Maintain accurate documentation in the Electronic Health Record system

  • Support community health activities such as National Health Center Week

Qualifications:

  • High School Diploma or equivalent required; 2+ years in social services preferred

  • Covered California Certified Enrollment Counselor (required)

  • Medicare Counselor Certification and Connect to Care Certification (required)

  • CPR certification required

  • Valid California Driver’s License, insurance, and reliable transportation

  • Bilingual preferred (Spanish, Punjabi, or Hmong)

  • Strong communication, organizational, and cultural competency skills

  • Proficient in Microsoft Office and EHR systems

Qualifications

COMMUNICATIONS SKILLS

  1. Must have neat and legible handwriting
  2. Must be able to interact with patients courteously and calmly
  3. Ability to communicate well with the public in a linguistically and culturally appropriate manner.

 

PROFESSIONAL KNOWLEDGE, SKILLS & ABILITIES:

  1. High school graduate or equivalent
  2. At least two years’ experience in community social service activities preferred
  3. CPR Certified
  4. Covered California Certified Enrollment Counselor required
  5. Medicare Counselor Certified per State Guidelines
  6. Connect to Care Certification through County Medical Services required
  7. Completion of training on Prescription Assistance Program, Every Woman Counts and Breast Cancer Detection Program
  8. Ability to travel to other locations as needed
  9. Valid California Driver’s License and proof of insurance
  10. Reliable method of transportation for required travel
  11. Excellent communication skills
  12. Bilingual preferred
  13. Ability to relate to and work with people of all ages, social, cultural and ethnic backgrounds
  14. Demonstrate clear knowledge of Ampla Health’s clinic structure, standards, procedures and protocols
  15. Computer Literate (Microsoft Office)
