Ampla Health is seeking a dedicated Community Promotions Worker to serve as a vital link between healthcare providers and the community. Under the supervision of the CCSO, this role promotes access to medical and dental care, provides health education, and assists individuals with enrollment in various health coverage programs.

Key Responsibilities:

• Provide patient education, translation, and transportation assistance.

• Inform and counsel community members about Ampla Health programs and services.

• Assist patients with scheduling, eligibility applications (Medi-Cal, CMSP, etc.), and enrollment forms.

• Act as a liaison between Ampla Health and community organizations.

• Maintain patient confidentiality and ensure compliance with HIPAA regulations.

• Attend community events, trainings, and cultural competency classes.

Qualifications:

• High School Diploma or equivalent required.

• Two years of experience in community outreach or social services preferred.

• Current CPR certification required.

• Valid CA driver’s license, insurance, and reliable transportation.

• Excellent communication and interpersonal skills.

• Bilingual (English/Spanish) preferred.

Physical Requirements:

• Ability to lift up to 40 lbs. and travel between locations as needed.
