This rewarding role offers the opportunity to directly impact the lives of individuals with developmental disabilities while building a fulfilling career in human services. This role may support either Children or Transition/Adult programs depending on assignment.

- Note for San Andreas applicants: Your caseload location will be based on where you live (Tuolumne, Amador, or Calaveras).

What You’ll Do:
- Be a Champion for Others
- Advocate for the rights and services of individuals with developmental disabilities. Empower people and families to navigate systems and access life-changing support.
- Connect People to Resources
- Coordinate essential services such as healthcare, education, housing, and vocational programs that improve quality of life.
- Build Meaningful Relationships
- Work directly with individuals, families, schools, and community partners to promote independence and inclusion.

Key Responsibilities:
- Case Management & Planning
- Develop, coordinate, and monitor Individual Program Plans (IPPs)
- Facilitate team meetings and periodic reviews
- Documentation & Reporting
- Maintain detailed and timely case notes
- Complete evaluations, service authorizations, and Medicaid documentation
- Advocacy & Support
- Provide education, advocacy, and crisis support
- Act as liaison between clients, providers, and agencies
- Professional Collaboration
- Participate in committees and cross-agency initiatives
- Maintain positive relationships with service partners
- Emergency Response
- Serve as Officer of the Day and participate in After-Hours Response rotation
- Data & Analysis
- Collect and report data for surveys, assessments, and audits
