About the job

Our Clinicians provide care to nursing home residents enrolled in an Institutional Special Needs Plan. You will provide primary care and acute care management for ISNP-enrolled patients in assigned nursing home(s) in collaboration with an RN Care Coordinator. Our emphasis is on proactive and preemptive care through general health management, increased availability of on-site clinicians, and early identification of changes in condition to avoid unnecessary hospitalizations and ED visits. The NP establishes individualized care goals for each member and facilitates communication between members, families, providers, and facility staff, and establishes advance care preferences with each member. We strive to provide education about disease progression, address problems sooner in the disease process, and manage members in the skilled nursing facility whenever possible.

Qualifications

  • Master's Degree in Nursing (MSN)
  • Texas State licensure as a Nurse Practitioner
  • Current certification as a Nurse Practitioner
  • DEA Prescriptive License
  • Ability to work independently and as part of a team

Qualifications

Licensing Requirements

RN and APRN-CNS (or equivalent) in Texas, or Compact License with Texas privileges 

Experience Requirements

The NP is licensed as both a Registered Nurse and an Advanced Practice Registered Nurse in the state of Texas. The NP maintains an up to date Board Certification in his/her specialty.  The NP is responsible for maintaining a Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors and for maintaining Texas Board of Nursing ongoing educational requirements for licensure and re-certification. The NP will comply with all State and Federal guidelines required to practice as a licensed Advanced Practice Registered Nurse, including maintaining an active RN and APRN license.  The Company will provide Professional Medical Malpractice Liability Insurance to the NP.
