Job Summary
The Admissions Coordinator serves as the initial point of contact for patients and families entering hospice and/or palliative care. This role is responsible for coordinating the admission process, ensuring a compassionate, timely, and efficient transition to hospice/palliative services. The coordinator acts as a liaison between referral sources, patients, families, and the interdisciplinary hospice team.

Hours: Monday – Friday, 8:00am – 4:30pm
Salary: $70,000-$78,000/year
Reports to: Clinical Services Manager

Responsibilities
- Receives and processes all hospice and palliative care referrals in a timely, professional and compassionate manner.
- Coordinates and schedules admission visits with patients, families, and admissions/interdisciplinary team members.
- Verifies hospice eligibility criteria and collaborates with Physicians for certification.
- Provides patients and families with information about hospice services, benefits and expectations as appropriate.
- Communicates with referral sources, Physicians, hospitals, nursing homes, and case managers to gather necessary clinical and administrative documentation.
- Maintains accurate and complete admission records, including H&Ps and orders to admit from Primary Care Physicians.
- Follows up on patients not admitted and patients who have been discharged based on established protocols.
- Enters patient medications into patient chart as requested.
- Monitors and tracks referral and admissions metrics and contributes to ongoing process improvements.
- Supports Marketing/Healthcare outreach efforts by building strong relationships with referral sources, promoting the hospice mission.
- Receives and triages incoming nursing calls from patients, caregivers and providers.
- Maintains daily census information in EMR and communicates changes as needed to clinical staff.
- Assists with maintaining the medical record including applying identification stickers to new admission charts, thinning of charts, and tear down and closing of charts upon death or discharge.
- Print PDC reviews every Monday morning.
- Completes initial admission chart audits.
- Acts as a backup resource to complete Admissions as needed.
- Makes changes to the plan of care for Hospice Aides as requested.
- Coordinates out of the area transfers and assists in the preparation of travel contracts.
- Assists with filing patient plan of care, MD orders, initial certifications, recertifications, out of hospital DNR forms and private insurance updates.
Qualifications
- Current Iowa State license as a Registered Nurse required. Bachelor’s degree in nursing (BSN) preferred.
- Strong technical skills with proficiency in Microsoft Office required.
- Minimum 3-5 years of clinical nursing experience in related healthcare setting required.
- At least 2 years of experience in MDS coordination, case management or healthcare operations leadership preferred.
- Experience working in a fast-paced clinical or administrative setting with multiple priorities and frequent interruptions preferred.
- Strong communication and customer service skills required.
- Experience working with electronic medical records required.
- Skilled at working both independently and collaboratively with a team.
- Current Iowa driver’s license, auto insurance and reliable vehicle required.
Physical/Other Requirements
- Ability to move 25lbs with our without accommodation.
- In an 8-hour day, employee may remain in a stationary position 3-7 hours and move about inside CVH site offices/locations, meeting spaces, and client residences 1-5 hours (or vice versa).
