Description

Physician Affiliate Group of New York (PAGNY) is adding a Physician Assistant - Heart Failure to the group at NYC Health + Hospitals/Jacobi and North Central Bronx. Jacobi Medical Center is a 457-bed, Level I Trauma Center and North Central Bronx Hospital (NCB) has 213 licensed beds. Jacobi Hospital has been recognized by the U.S. News & World Report as a High Performing Hospital for Heart Failure and have received multiple accolades Get With The Guidelines® Heart Failure Gold/Silver Achievement Awards from the American Heart Association.

 

The hospital maintains a strong academic affiliation with Albert Einstein College of Medicine to maintain its high healthcare delivery standards. In addition Jacobi Hospital through the Department of Cardiology is a major teaching site for the cardiovascular fellows from AECOM/Montefiore Medical Center.

The heart failure team is adept in managing complex cases, access previously unattainable therapies from medications to ambulatory hemodynamic monitoring with Cardiomems® and for the sickest of patients we have been able to advocate for durable mechanical support and heart transplant. The department of cardiology also has access to multimodality imaging to help aide in management such as echo, CCTA, nuclear imaging, cMRI, and PET-CT.

Opportunity Details:

  • Interested in working with a growing heart failure team (including advanced heart failure and transplant physician and other cardiologists, NP/PAs, pharmacists, cardiology fellows, hospitalists and resident house staff).  
  • Interested in both inpatient acute management of decompensated heart failure and transition of care to the outpatient ambulatory setting.
  • Ability to conduct a comprehensive assessment of a patient with heart failure including taking a detailed health history, identifying triggers/etiology for heart failure, and performing a physical examination with comprehension of the range of euvolemia to decompensated heart failure, and identify concerns for impending cardiogenic shock.
  • Familiar with diagnostic tests such as ECG (including basic interpretation of an ECG), ischemic evaluations (from stress tests to CCTA/LHC), and some familiarity in reviewing test results such as echo, holter, stress test and ambulatory hemodynamic monitoring with Cardiomems®.
  • Should have understanding in the two major forms of heart failure – HFrEF and HFpEF and the specific management of both using guideline directed medical therapy including titration of medical therapy. 
  • Should be passionate and able to educate patients and their families about heart failure and its management. 
  • The provider will work closely with the heart failure physician and other cardiologists/healthcare providers and should have an excellent track record of clear communication and collaboration as this is vital in implementing treatment plans.

 

Qualifications

  • Current certification/admissibility issued by the NCCPA (National Commission on Certification of Physician Assistants). 
  • Current NYS registration as a Physician Assistant. 

Wages and Benefits Include: Annual Base Salary: $127,308* to $169,744* (based on years of experience) based on a 40-hour work week.

The annual total value of the compensation package is estimated at $140,039** to $186,718** (based on years of experience), which includes the baseline salary, 401(k) contribution, and other factors as set forth below:

  • 401(k) Company Contribution (subject to IRS contribution limits):
    • Employees are immediately vested in a 3% company contribution of base earnings. No employee match is required.
    • After one year of service, employees receive an additional 7% company contribution of base earnings. No employee match is required.
