Peer Specialist (BHCC) CASE Manager

Position Summary:

Under the direct supervision of the Coordinator of Paraprofessional Services, the BHCC Peer Case Manager provides a broad range of services to clients discharging from the Advantage Behavioral Health Crisis Center and their families including, but not limited to outreach, engagement, peer support, and case management. Using their own experience of recovery, the BHCC Peer Case Manager serves as a client advocate and models competency in recovery and wellness. The BHCC Case Manager follows up with individuals by phone and face to face in the community with the goal of increasing client follow-up and engagement with aftercare services. This position serves the ten-county Advantage catchment area.

Job Responsibilities and Performance Standards:

Service Delivery

• Follows Prescriber’s Orders and Treatment plans regarding services to clients and frequency of services.
• Meets with clients/clients’ families to address issues and work on skills as listed in treatment plan goals
• Assists clients in creation of Wellness Recovery Action Plans
• Provides social and psychological support to clients through listening and discussing problems and progress as necessary.
• Assists clients with activities of daily living, as needed.
• Utilizes peer training to support clients in living their own lives and directing their own recovery and wellness process.
• Supports clients in identifying their personal goals and objectives for recovery and wellness and in identifying strengths of the client that will aid in reaching them.
• Completes required documentation, such as progress notes, referrals, memos to chart, etc. timely and according to agency policies and procedures
• Documentation of face-to-face sessions, phone contacts, and all collaborative communication with each client and/or service provider/agency in the EHR per policy.
• Complies with regulations on confidentiality of client information as specified by established policy.
• Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services
• Provides appropriate verbal and written information to other staff, health care professionals, clients and families regarding medical issues, social services, problems and needs.
• Provides assistance, when needed, with scheduling medical appointments and arranging transportation. Transports clients to appointments as needed.

Collaboration

• Serves as a resource to other staff by identifying program elements that foster recovery for clients by using their personal experience of mental illness and what makes recovery possible.
• Provides appropriate information and/or input in meetings pertaining to case management and case collaboration.
• Collaborates with service providers in addressing concerns and treatment planning changes or additions.
• Works cooperatively with other community agencies which may be serving the same client(s)
• Attends staff meetings, conferences, workshops, or other meetings as required by the supervisor.

Reporting

• Tracks data for key performance indicators to demonstrate compliance with grant objectives. For example, tracks the number of clients served and the number of clients who engage with planned/scheduled aftercare treatment upon discharge.

Training

• Will attend and successfully complete all required trainings including, but not limited to, DBHDD Paraprofessional trainings, Advantage required trainings, BHCC required trainings, and grant-related trainings such as TREM and Seeking Safety.
• Attend scheduled Peer Consultation and Support Group, as scheduled.

Minimum Qualifications: 

Candidates must have a diagnosis of mental illness, addiction, or a dual diagnosis of mental illness and addictive disease and a strong desire to identify themselves as a person with mental illness (current or former consumer of mental health services).

• Applicants must hold a GED or High School diploma and be at least 18 years of age. You may be requested to provide a copy of this document.
• In addition, applicants must demonstrate strong reading comprehension and written communication skills
• Applicants must have demonstrated experience with leadership, advocacy, or governance, and be well grounded in their recovery (per CPS and CARES guidelines).

Preferred Qualifications: 

Bachelor’s degree in a related field.

A commitment and dedication to working with adults with severe and persistent mental illness and a willingness to provide treatment to clients in their homes and other community settings.

Certificates, Licenses, Registrations Required:

Must be a certified peer specialist to retain this position.
