POSITION SUMMARY:  Provides a variety of medical services to help individuals who have been injured or physically affected by illness to recover or improve function. The physical therapist must be able to evaluate a patient’s condition and devise a customized physical rehabilitation and treatment plan and modify as needed.  Primarily works in primary department listed above but agrees to cover in other areas of SMH as needed/requested. This position is responsible for any patient requiring therapy throughout the organization (Inpatient, Outpatient, Swing-bed, Homecare and Hospice).
 DUTIES & RESPONSIBILITIES:
Direct patient care, involving both evaluation and treatment of referred patients.
Comprehensive and Timely documentation of evaluation(s), discharge daily notes and progress notes.
Communication with providers and others team members involved in the patient’s care as needed or required.
Practice therapy consistent with the APTA code of ethics and the Michigan State practice act.
Assist with Quality Improvement projects as requested.
Attend departmental meeting, hospital inservices, and continuing education as directed.
Must maintain patient confidentiality and abide by hospital wide policies of conduct.
Supervision of physical therapist assistants.
Provide supervision/evaluation of students as assigned by the CCCE.
Commits to the Behavioral Standards of SMH.
Demonstrate appropriateness in meeting objectives in age-specifics.
Performs other duties as assigned by the department manager.
