POSITION SUMMARY:  The Behavioral Health Therapist is a professional who will screen, assess, diagnose and treat co-occurring mental health and substance use disorders. They work with individuals, groups, and families of all ages to help them manage and overcome their mental health concerns.

DUTIES AND RESPONSIBILITIES:

  • Maintain clinical caseload agreed upon by clinic director and CEO.  This includes providing quality patient assessments to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment and or life situation.
  • Develop individualized treatment plans based on patients’ preferences, values, goals, and needs.
  • Utilize evidence-based methods to provide group and individual treatment to patients and families.
  • Collaborates with community leaders on projects/initiatives to further develop behavioral health resources in the local community and within SMH. 
  • Process referrals to psychiatric providers or other practitioners for psychiatric testing or other mental health or behavioral health care needs.
  • Provide consultation to and coordinate care of patients with Family Practice primary care staff. 
  • Complete all documentation as needed and required: including assessments, treatment plans, progress notes, discharge summaries etc.
  • Maintain confidentiality in compliance with HIPAA and RHC regulations.
  • Develop a collegial relationship with RHC and SMH staff and attend requested staff and other meetings.
  • Demonstrates appropriateness in meeting objectives in age-specifics.
  • Perform other duties as may be assigned.

Qualifications

One of the following:

        Master’s Degree in Social Work, with a current Michigan license as a Licensed Social Worker (LMSW or LLMSW) or

        Master’s degree in Professional Counseling with a current Michigan license as a Licensed Professional Counselor (LPC)

Experience working with patients who have co-occurring mental health, substance abuse, and physical health problems.

Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.

Willingness to work with diverse populations

Heartsaver CPR Certified (obtain upon employment)
