Job Summary

The Older Females Program Case Manager provides supportive services to 20 single women ages 21-24 at a shelter site in the Bronx funded by DYCD. Reporting to the Program Coordinator of the shelter site, the Case Manager is an energetic, creative, and diligent member of a team that includes resident advisors (who also have cooking responsibilities) and is a diligent member of a team that includes case managers and specialty support staff. The Case Manager will provide comprehensive case management services to the youth who are on their caseload, including: creation of an individual service plan, coordination of services, personal counseling, advocacy, monitoring of the efforts and challenges of the youth in completing goals, and celebrating goal achievement.

DUTIES AND RESPONSIBILITIES

 (Responsibilities to include, but not limited to the following)

  • Case Management
    • Provides integrated counseling and case management services that effectively engage young adults and promote personal growth and wellness, housing stability, and ongoing accomplishment of service plan goals.
    • Collaborates with youth to assess strengths and challenges and develop an individual service plan that reflects the youth's progress and stated goals.
    • Meets with youth regularly to determine progress with goals identified in the ISP.
    • Advocates on behalf of youth with outside service providers and other collateral services.
    • Creates and maintains confidential records, charts, and logs for youth, documenting all relevant information from individual and group interventions and collateral services.
    • Ensures that youths' files are compliant with all regulatory and other requirements.
    • Completes incident reports per agency policies.
    • De-escalates crises; attends to emergencies following agency protocol, as needed.
  •  Team Participation
    • Works as a collaborative member of the team, ensuring that all young women have access to high-quality services and are living in a safe and compassionate community.
    • Participates in regular case conferences. Ensures team members are fully apprised of important information.
    • Actively participates in staff team meetings, individual supervision meetings, and meetings with the young women.
    •  Assists with audit preparation and dry and live audits of the program.

Qualifications

KNOWLEDGE, SKILLS, & ABILITIES

  • Bachelor’s degree preferred. 
  • If no bachelor's degree is required, a high school or GED is required, and one (1) year of experience in a residential setting working with adolescents is preferred.
  • Strong skills in leading groups, taking the lead in collaborating with internal and external departments
  • Demonstrated knowledge of and experience working directly with young people through approaches emphasizing resiliency, strength-based youth development, and trauma-informed care required.
  • Strong multi-tasking skills with demonstrated ability to manage multiple projects simultaneously are required.
  • Excellent verbal, written, and problem-solving abilities and an upbeat demeanor are required.
  • A strong focus on child protection and ensuring the safety of residents is required.
  • Experience working with diverse groups is required.
  • Overall flexibility in work schedule required, including weekends, evenings, and holidays if needed.
  • Experience utilizing evidence-based and other social service models preferred.
