Position Overview:
We are seeking a dedicated and compassionate Registered Dietitian (RD) to join our team in a social service program aimed at addressing Social Determinants of Health (SDOH) related to nutrition. Individuals in this position are responsible for nutrition education, conducting nutritional assessments, and connecting clients with essential resources to improve their health and wellbeing. This role plays a key part in helping underserved populations overcome barriers to healthy eating and nutrition.
What you will do:
- Provide individualized and group nutrition education sessions to community members, focusing on accessible, affordable, and culturally appropriate nutrition solutions. Topics may include healthy eating on a budget, cooking demonstrations, improving food access, and information sessions about available community resources.
- Conduct comprehensive nutrition assessments to evaluate the dietary needs and habits of individuals. Use assessment findings to develop tailored nutrition care plans that address specific health concerns related to SDOH such as food insecurity, limited access to nutritious foods, and other social and environmental factors.
- Assist with community-clinical linkages and internal referrals to other NARA programs as appropriate
- Maintain accurate records of client assessments, interventions, and outcomes.
- Collect data and provide regular reports required for Social Services Program grant continuation and track effectiveness and program impact.
- Manage SDOH nutrition referrals in a timely manner
- Assist with the coordination of care by taking part in scheduling of appointments, coordinating referrals, and sharing or transferring information with the patient's care team
