Compensation:

$24.04-26.44/hr based on experience

Position Description:

Serve as staff social worker, coordinating needs assessments and advocating for the overall wellbeing and quality of life of Interfaith senior clients. Coordinate Senior Activities at Senior Living Complexes as part of Interfaith Senior Wellness Programs. Coordinate SEW Seniors Eating Well grocery delivery program as part of Interfaith Senior Care Programs, oversee activities and grocery delivery at Windvale Pines

Primary duties

  • Provide comprehensive needs assessments, including home evaluation and assessing activities of daily living to maximize client ability to continue living independently and engaged in community. Develop individual client plans for services provided by Interfaith.
  • Stay current on all public and nonprofit social services programing and services for senior adults and trends in gerontology.
  • Facilitate social and recreation activities for senior adults at Senior Living Complexes (Holiday events, birthday celebrations, games & recreation, lunch & learn workshops, bingo).
  • Collaborate with SCCC Activities Program Coordinator to plan and shop for supplies needed for all programs and events.
  • Coordinate Worship Services in collaboration with Interfaith member congregations as needed.
  • Coordinate SEW Seniors Eating Well, grocery delivery program.
  • Facilitate monthly activities at Windvale Pines apartment complex.
  • Assist with SOS, Serving our Seniors and SON, Serving our Neighbors programs biannually.
  • Assist in Holiday GIVING Program for senior adults.
  • Maintain accurate client records in Client Track database.
  • Provide information and referrals for senior adults, caregivers, and the community at large.
  • Provide special events support to all Senior Services programs as needed.
  • Work with Volunteer Coordinator to provide and coordinate volunteer opportunities for SS programs, including data entry of all volunteer hours.
  • Support I & R coverage as needed.
  • Assist in Interfaith Disaster Response efforts.
  • Other duties as assigned.

Qualifications

Qualifications:

  • Bachelor degree, licensed social worker, and previous social services/nonprofit experience required
  • Must be insured driver to use company vehicle.  

  Preferred:

  • Experience with the intersection of geriatric, disabled and neurodiverse populations
  • Live within 15 miles of 77381 area
  • Bilingual (English/Spanish)

Knowledge, Skills and Abilities:

  • Strong organizational, customer service and interpersonal skills
  • Ability to manage multiple tasks
  • Computer proficient

Travel:

Requires occasional local travel with mileage reimbursement. Employee must comply with Interfaith’s Safe Driving policy.
