Position Summary

The Youth and Young Adult (YYA) Case Manager supports individuals ages 18–24 in the agency’s Youth and Young Adult Transition in Place (YYA TIP) program. As a member of the Housing & Shelter team, the Case Manager provides comprehensive case management services—including housing navigation, income development, and connections to behavioral health and medical care—to young adults transitioning out of homelessness. This role is dedicated to promoting housing stability and self-sufficiency, with the ultimate goal of ensuring that homelessness is brief, rare, and nonrecurring.

 

Some of the duties include:

  • Provide engagement and ongoing case management services to clients ages 18–24 enrolled in the Youth and Young Adult Transition in Place (YYA TIP) program
  • Connect clients to appropriate resources and community partners, including assistance with obtaining identification, accessing public benefits, and linking to mental health, substance use disorder services, and housing interventions
  • Support clients in increasing income to sustain rental responsibilities by the end of the program through referrals to job training programs, employment opportunities, and/or assistance with SSI/SSDI applications
  • Ensure linkage to Cuyahoga County’s Coordinated Entry, if not assessed prior to Shelter entry
  • Assist in arranging and/or providing transportation to critical destinations (e.g. housing, medical, employment)
  • Support participants with move-in coordination and relocations, as needed, to ensure a smooth transition into or between housing units
  • Ensure education and guidance on essential daily living skills to help participants successfully maintain housing and live independently
  • Coordinate with YYA TIP partners including property liaison team, FrontLine Service, Family Promise, A Place 4 me, OHS
  • Maintain participant data and statistical information, client database records, Housing Assessments, and progress notes in HOPE and User Form data tracking systems

 

Qualifications

Our ideal candidate has a High School Diploma or GED; Bachelor’s Degree preferred. Some experience working with people experiencing homelessness, veterans, people with substance use disorders, people with re-entry backgrounds, and/or other relevant social service populations, preferred. A Valid Ohio Driver’s license and proof of insurance, required
