1. Graduate from a two-year college level program approved by the American Physical Therapy Association. Â Â
2. Currently licensed, registered or certified in the state(s) in which practicing. Â Â
3. Two (2) years’ experience preferred.  Â
4. Acceptance of philosophy and goals of Agency. Â Â
5. Ability to exercise initiative and independent judgment. Â Â
RESPONSIBILITIES: Â Â
1. Understands and adheres to established Agency policies and procedures. Â Â
2. Provides physician prescribed physical therapy under a plan of care established by the PT. Â Â
3. Improves or minimizes residual physical disabilities of the patient. Â Â
4. Returns the individual to an optimum and productive level within the patient’s capabilities.  Â
5. Participates with all other health care personnel in patient care planning. Â Â
6. Performs all skilled procedures as ordered by physician and according to the plan of care established by theÂ
PT. Â Â
7. Consults with PT regarding change in treatment. Â Â
8. Instructs patients and family members in home programs and activities of daily living. Â Â
9. Participates in in-service programs and presents in-service programs as assigned. Â Â
10. Participates in QAPI activities as assigned. Â Â
11. Attends all patient care conferences as scheduled. Â Â
12. Prepares medical records, progress notes and updates care plans for each patient visit in a timely manner asÂ
per Agency policy. Â Â
13. Performs services planned, delegated, and supervised by the PT. Â Â
14. Assists in preparing clinical and progress notes. Â Â
15. Participates in educating the patient and family. Â Â
16. Performs maintenance therapy services under a maintenance program established by a qualified therapist,Â
when acting within the therapy scope of practice defined by applicable state licensure laws.Â
WORKING ENVIRONMENT: Â Â
Works indoors in Agency office and patient homes and travels to/from patient homes. Â
Job Relationship:Â Supervised by Registered Physical Therapist/Clinical Director

- Ability to participate in physical activity
- Ability to work for extended period of time while standing and being involved in physical activity.
- Heavy lifting.
