Description

The Wellness Manager is responsible for planning, implementing, and evaluating health and wellness initiatives that serve the local community. This role focuses on addressing health disparities, promoting preventive care, and increasing access to wellness resources through culturally responsive and inclusive programming. The Wellness Manager collaborates with internal departments—including the Communications & Engagement Department—as well as external partners and community members to build relationships and ensure programs align with the organization’s mission and the needs of the populations served.

  • Design and lead wellness initiatives that promote physical, mental, and emotional health for community members. Includes initiatives related to preventing or improving chronic health conditions, including diabetes and pre-diabetes
  • Serve as liaison and provide oversight to PCC’s contracted community partners related to wellness initiatives, including the PCC Farm
  • Coordinate and manage outreach events, workshops, support groups, and wellness campaigns on topics such as nutrition, physical activity, chronic disease prevention, stress management, and health literacy.
  • Serve as direct supervisor for select staff associated with health and wellness initiatives
  • Partner with clinical teams, community health workers, and local organizations to expand program reach and impact.
  • Identify community needs and gaps in wellness services through surveys, focus groups, and community feedback.
  • Collect and analyze data to evaluate the effectiveness of wellness programs and report outcomes to leadership and funders.
  • Manage program budgets, grant deliverables, and reporting requirements as needed.
  • Develop culturally relevant materials and resources to promote community wellness and engagement.
  • Promote a sense of community by fostering welcoming, safe spaces where connection, fun, and joy are recognized as vital components of wellness.
  • Nurture the social aspects of health by creating opportunities for community members to connect meaningfully through inclusive and culturally responsive programming.
  • Represent the organization at community meetings, coalitions, and public health collaborations; help determine appropriate staff representation and serve as a delegate when needed.
  • Partner with the Communications & Engagement team and leadership to identify and support opportunities for community connection—including meetings, partnerships, and events—and maintain flexibility to attend community-based events during evenings and weekends as needed.

 

Qualifications

Experience/Training: 

  • Bachelor’s degree in public health, Health Education, Social Work, Community Health, or a related field required
  • 2+ years of experience developing and implementing community health or wellness programs, preferably in a non-profit or public health setting.
  • We value candidates who bring lived experience or a deep connection to the communities we serve. Knowledge of health disparities, social determinants of health, and culturally responsive program design.
  • Strong organizational and communication skills, including public speaking and facilitation.
  • Bilingual or multilingual skills are a plus, especially in languages commonly spoken in the service area.
  • Experience with program evaluation and data collection methods preferred.

Physical Demands:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to stand, bend and stretch.
  • Ability to travel and maneuver between site locations if needed.
  • Ability to lift 20 pounds.
  • Ability to type for long periods of time.
  • Ability to be on-site at least 5 days a week.

Language Skills:

  • Ability to read and write proficiently using the English language.
  • Ability to interact and speak effectively in interpersonal situations.
