Job Summary: The Clinical Refill Coordinator is responsible for managing prescription refill requests for patients across assigned clinical teams. This role ensures timely, accurate, and safe processing of refill requests in accordance with provider delegation, clinical guidelines, and organizational protocols. While medication refills will be the primary focus of this position, additional responsibilities may be assigned at the discretion of the Director of Team Nursing based on organizational needs.
Essential Duties and Responsibilities:
- Review and process prescription refill requests using established clinical protocols and EMR workflows.
- Work under provider-delegated order sets to authorize refills when appropriate.
- Communicate with pharmacies, patients, and clinical team members regarding refill status, medication changes, or questions.
- Flag refill requests that fall outside protocol and require provider review.
- Document all medication refill activities accurately and timely in the electronic health record (EHR).
- Participate in medication refill training and maintain competency in protocol use.
- Monitor the team “Orders” desktop bucket regularly throughout the day for new refill requests.
- Collaborate with providers and nurses to ensure continuity of care and adherence to medication safety standards.
- Identify and escalate any patient safety or compliance concerns related to medications.
- Assist in evaluating the sustainability and efficiency of the medication refill workflow.
- Collaborate with the Director of Team Nursing to explore the development of a support structure, including identifying opportunities to train and delegate appropriate tasks to additional staff under your oversight to enhance refill capacity.
- Contribute to workflow improvements, staff role definitions, and pilot models for expanded refill team functions.
- Monitor, analyze, and support improvement related to medication adherence and prescription focused measures in collaboration with Quality Improvement.
- Perform other tasks as assigned by the Director of Team Nursing, which may include general nursing support, administrative tasks, or clinical documentation assistance.
